Job Overview

The Clinical Care Coordinator is a Registered Nurse (RN) who manages a specific panel of patients identified as having rising risk or high risk. The Clinical Care Coordinator is involved in the coordination of care for a designated panel of patients across the continuum of care, as well as identify and monitor patients with rising risk. The Care Coordinator utilizes the nursing process and serves as the leader and driver of care coordination efforts for their panel of patients, in partnership with the multi-disciplinary team. The coordinator ensures that the care provided is patient and family centered, evidence-based, cost-effective, holistic in nature, and leads to better outcomes. The coordinator is responsible for partnering with the patient and family for seamless transitions in care between the hospital, home, school and community.

Shift: 9 hr shifts, 4x per week. Day off tbd. Opportunity to work hybrid after 6 months and outstanding performance.

Qualifications

Education: Bachelor of Science in Nursing (BSN)

Experience: Four years nursing experience (RN)

Certification(s):

  • BLS/CPR from the American Heart Association with at least 6 months left before expiration is required upon hire.
  • Achievement and maintenance of certification in one of the two categories:
    • Certified Case Manager (CCM) through the Commission for Case Manager Certification (CCMC), Accredited Case Manager (ACM) through the American Case Management Association (ACMA) or certification in Care Coordination and Transition Management (CCCTM) through the American Nurses Credentialing Center (ANCC).
      • A plan to obtain certification will need to be in place with the team member's direct leader within 6 months of hire with the expectation that the certification exam will be taken once all accrediting body eligibility requirements are met.
    • ANCC or Magnet approved clinical specialty certification inclusive of care coordination activities as approved by the Director of Patient Care Services.
      • A plan to obtain certification will need to be in place with the team member's direct leader within 6 months of hire with the expectation that the certification exam will be taken once all accrediting body eligibility requirements are met
  • Successful completion of required internal Care Coordination training within six (6) months of hire or transition into role.

Licensure(s): Registered Nurse (RN) licensure in the state of Colorado is required.

Professional Membership(s): Active membership in a professional nursing or healthcare organization is required.

ESSENTIAL FUNCTIONS

An employee in this position may be called upon to do any or all of the following essential functions. These examples do not include all of the functions which the employee may be expected to perform.

  1. Utilizes care coordination process by:
  • assessing patient/family needs in hospital, home, school, or other community location
  • establishing and implementing a care plan in partnership with the patient/family and multi-disciplinary team to proactively support the patient/family
  • linking families to relevant community providers/services, home-based services, school-based services, parent support and advocacy groups
  • documenting within the standardized care coordination tools and maintaining the Care Plan
  • evaluating patient/family needs and outcomes to continuously update the Care Plan
  • tracking care coordination interventions and outcomes.
  1. Responsible for complex panel of patients supporting patients/families in their plan of care across all settings (hospital, home, school, community).
  2. Serves as a point of contact, advocate, navigator, and informational resource for patient, family, care team, payers, and community resources. Facilitates related meetings as needed.
  3. Collaborates with patients, families, and providers to influence, and enhance adherence to, an effective plan of care based on mutually developed goals in an environment that fosters shared decision making.
  4. Assesses for patient/family needs in a comprehensive, holistic manner including identifying and addressing unmet medical, social, mental/behavioral health needs.
  5. Uses reports to proactively manage care gaps, over/under utilization, understanding/compliance with Care Plan, and progress towards shared goals.
  6. Provides education and coaching to patients and families to improve health literacy and work towards self-management of their health.
  7. Collaborates with the patients/families to develop a relationship that supports open communication and creation of a care plan that supports the level of support desired by the patient/ family and will lead to optimal outcomes.
  8. Facilitates patient access to appropriate providers as well as other care coordination support, including, but not limited to, medication acquisition and management, procedures/tests, appointments, and referrals.
  9. Participates in practice transformation efforts that support improved care coordination across the continuum through sharing knowledge, acting as role models, and providing technical assistance.
  10. Facilitates transitions in care between the hospital, home, school, and community and ensures that the child's optimal care outcomes are not impacted by the transition.
  11. Participates in the development and revision of policies and procedures, as well as with quality improvement and research activities, impacting care for the designated patient population.
  12. Participates in internal and external learning community and professional governance committees focused on care coordination or population health.

Salary Information

Annual Salary Range (Based on 40 hours worked per week): $78,659.24 to $117,988.86
Hourly Salary Range: $37.82 to $56.73
