CTL for Lexington and Bedford locations. Management of Bedford office and staff. Primary work location is Lexington with occasional travel to Bedford location. Leadership on-call responsibilities.

Serves as a team leader, clinical resource person, role model, mentor, and facilitator for staff and department operations. Is responsible and accountable to lead staff; resolve routine issues, and create a positive, productive, and fiscally responsible team. Remains aware of organizational developments and initiatives and implements as needed for their team. Refers complex or potentially significant issues to manager/supervisor, including but not limited to; issues regarding compensation, work hours, working conditions, hiring, and formal corrective action. Initiates and facilitates effective open two-way communication and resolution of work site operational issues that support service area goals and objectives. In addition to the lead responsibilities, maintains clinical competencies and provides professional care to patients of all ages in response to agency needs.

  • Assesses, plans, implements, and evaluates patient care in order to achieve optimum patient outcomes for patients as appropriate to service area.
  • Utilizes leadership principles in mobilizing the Carilion Home Care/Hospice team to achieve positive patient outcomes. Follows-up on matters to ensure they are successfully resolved.
  • Assists manager/supervisor in assuring staff competence and contributes to staff performance development. Identifies areas of improvement.
  • Assists with the coordination of work flow process of the home care/hospice team.
  • Collaborates with manager/supervisor regarding fiscal management. Monitors staff productivity overtime, mileage, and supply usage to ensure appropriate resource allocation and expense control.
  • Assists with other agency responsibilities including but not limited to; implementing and managing daily activities of the office, maintaining regulatory compliance, reviewing documentation, ICD-10 coding, monitoring of face-to-face documents, re-certifications, participating in performance improvement and other quality improvement initiatives. Ensures Joint Commission, Medicare and other third party payer guidelines are implemented and followed. Ensures compliance with policies and procedures.
  • Conducts individual case conferences and participates in interdisciplinary team meetings to facilitate effective two way communications. Listens to staff member's needs and concerns and resolves items brought to their attention. As appropriate, follows up with staff members on identified issues and concerns and reports to manager/supervisor as needed. When indicated, Initiates corrective action policy by employee coaching. Promotes positive work environment by holding staff accountable for their conduct, while using effective interpersonal skills. Demonstrates and encourages SBAR communication. Performs other duties as assigned.
  • Continuing education guidelines support the concept that learning is a continual process. For the Clinical Team Leader (CTL) job position the minimum requirement of continuing education is 20 hours annually.
  • Participates in special hospice events (e.g. grief camps, memorial services, etc.)
What We Require:

Education: Graduate of accredited Registered Nurse program. Bachelor's degree preferred.

Experience: Five years of clinical experience required. Demonstrated strong leadership skills, supervisory experience preferred. Home Health/Hospice experience preferred.

Licensure, certification, and/or registration: Currently licensed as Registered Nurse in the state of Virginia or approved compact state. Must have valid driver's license, automobile insurance, and personal vehicle for making home visits.

Life Support: Must obtain/maintain AHA BLS-HCP.

Other Minimum Qualifications: Demonstrates leadership qualities at current level of practice, good interpersonal and communication skills, and teambuilding skills required. Must successfully complete orientation and competency requirements. Keyboarding skills required for automated computer systems, including electronic medical records. Must be able to meet essential job functions.
