Position Description:

The Advanced Practice Provider (APP) works as a member of a multidisciplinary health care team who will contribute to excellence in program administration, patient care, patient safety, research, quality improvement, and teaching.

The Department of Medicine - Division of Geriatric and Palliative Care Medicine is seeking a Nurse Practitioner or Physician Assistant to support the Inpatient Palliative Care Services located Milwaukee, Wisconsin at Froedtert Hospital and Froedtert and Medical College of Wisconsin Clinical Cancer Center

The APP in this position will work in collaboration with palliative care physicians and other palliative care team members, to deliver palliative care and related health care services to patients and families. They will assist in the coordination of care for patients seen by the Community Health Division Palliative Care Consultation Service in the inpatient setting.

Primary Function/Responsibility

  • Collaborates with interdisciplinary health care teams in diagnosis and treatment of conditions requiring medical intervention in inpatient environment as a member of the Palliative Care and Hospice team.
  • Writes accurate and timely progress notes, procedure notes, and/or admission and transfer orders.
  • Order, perform, and interpret screening and diagnostic procedures and tests, and appropriately communicates and documents information.
  • Privileged to perform procedures beyond core as identified by the Palliative Care and Hospice services.
  • Facilitates and communicates with patient, family, and staff to promote continuity of care across the continuum of care.
  • Attend required training and meets key regulatory requirements, including those of the privileging institution, CMS, and the Join Commission.
  • Evaluates quality outcomes for the patients receiving care of this team through participation in the development of quality indicators and outcomes dashboard.
  • Acquires information on the most current development and trends in the Physician Assistant field and/or Nurse Practitioner field, through educational programs, in-services, professional associates, and current literature.
  • Provides guidance to home care and hospice staff. Establishes and maintains professional communication strategies with consulting provider and palliative team members.
  • Assists in development of training plans for pain management and end of life skills for nursing staff in inpatient, outpatient, and community settings. Utilizes training plans to provide training for the same.
  • Assists with educational efforts in symptom management and end-of-life issues for house-staff including fellows, residents, and medical students.
  • Acts as preceptor/educator for APN students.
  • Assists with and participates in committee work relevant to system management and end of life care.
  • Assists with Palliative Care Program development and continuous quality improvement related to Palliative Care and Hospice Care at Froedtert Hospital.

  • Performs comprehensive histories and physical exams, identifies normal and abnormal findings, including physical, cognitive, and psychosocial status.
  • Order, perform, and interpret screening and diagnostic procedures and tests, and appropriately communicates and documents information.
  • Develops pharmaceutical and non-pharmaceutical treatment plans.
  • Monitors effectiveness of plan of care.
  • Educates patients on disease and treatment plans.
  • Performs procedures as trained and appropriately credentialed.
  • Documents within the Electronic Medical Record (EMR) and completes other necessary documents.
  • Collaborates and/or consults with physicians and other health care providers as appropriate.
  • Educates, counsels, and assists patients and families regarding current health problems, health maintenance activities and preventive behaviors.

Minimum Education:

Advanced Practice Nurse: Master's Degree or higher in Nursing
Physician Assistant: Bachelor's Degree or higher in Physician Assistant Studies

Minimum Requirements:

Advanced Practice Nurse

Advanced Practice Nurse Practitioner

Wisconsin RN License

Wisconsin Advanced Practice Nurse Prescriber License

Adult Care NP and/or Geriatric NP preferred.

Will consider certification as Acute Care NP and/or Family Practice NP.

Additional Certification in hospice/palliative care or oncology nursing, preferred.

AHA ACLS, AHA BLS, DEA

Physician Assistant

Wisconsin Physician Assistant License

Certification by NCCPA

AHA ACLS, AHA BLS, DEA

Preferred Experience:

Five years prior experience as an Advanced Practice Nurse is preferred with at least 1-2 years in a clinic or hospital environment working with palliative care, hospice care, or hematology/oncology patients preferred.
