Job Summary:

The Medical Coordinator is responsible for assessing, facilitating and monitoring all components of client medical services. This includes but is not limited to: Transcribing medical information, assisting with medical assesments, providing support to the Lead and Assistant Lead Medical Coordinators and/or Regional RN, developing reports, acting as a liason between SWK medical department and contracted Primary Care Providers (PCP) etc. Additional responsibilities include ensuring compliance with Office of Refugee Resettlement (ORR), state licencing, and SWK policies and procedures.



Essential Functions:

    Advance typing and computer skills required along with ability to transcribed and understand medical information.
  • Maintain SWK's electronic health record and ORR's UAC portal by transcribing and updating regularly.
  • Responds to client's medical complaints and urgent medical needs immediately.
  • Consults with LMC ALMC, Regional RN, DCS Medical, and PCP when a medical need arises or for any medical guidance clarification.
  • For every youth, ensures prescribed and over the counter (OTC) medications are correct and in accordance with prescribing provider's orders.
  • Prepare youth for the initial medical examination (IME), take vital signs, and complete any needed medical questionnaires.
  • Assist the medical providers' staff with examination procedures when needed, such as patient screening, triage, and acting as a witness.
  • Dispense medication according to SWK's policy and procedure for assistance of self-administration of medication (ASAOM) and licensing standards.
  • Collect, maintain, and submit daily, weekly, monthly, quarterly, and annual reports to supervisor as required.
  • Assist with training new employees as directed by supervisor.
  • Maintain all medical inventory by reconciling daily.
  • Educates and counsels youth as directed by the PCP concerning medical status, disease, treatment, and prevention of disease and other health related topics.
  • Schedules and coordinates medical appointments as directed by contracted PCP. Ensures transportation and supervision is provided for these appointments.


Other Functions:

  • Able to react to change productively and handle other essential tasks as assigned.
  • Develop and maintain effective communication and working relationships with staff, co-workers, physicians, and UM team.
  • Collaborate, develop, and expand medical, dental, and other related services (e.g., labs, X-rays) with medical providers.

  • Maintain Health Information Privacy HIPAA.
  • Respond and comply to individual emergency medical needs immediately.
  • Maintain professional, confidential, and ethical standards guidelines by Southwest Key and other entities.


Qualifications and Requirements:

  • High School Diploma/GED
  • Minimum of 1 year employment experience working with children and/or adolescents
  • 6 months to one year of experience working in a medical setting (can include clinical hours worked in a certification program)
  • Advance typing and computer skills.
  • (Spanish/English). Ability to read, analyze, and interpret common medical reports and legal documents.
  • Cleared tuberculosis test results.
  • Immunization documentation: Tetanus, diphtheria, pertussis (Td/Tdap), Varicella, Measles, mumps, rubella (MMR), Hepatitis A and B.
  • Cleared background check from appropriate entity.
  • Cleared drug test results (for Texas programs only).
  • Comply with annual training requirements/licensure/certifications if applicable.
  • Valid Drivers' License.
  • Willingness to work a flexible schedule and be on-call evenings and weekends.

Must demonstrate exceptional:

REASONING ABILITY: Communication skills, ability to communicate and respond to common inquiries or complaints from both clients and staff from a variety of disciplines, addressing complaints and resolving problems.

Skills Preferred:

  • Certification as a Medical Assistant, Nursing Assistant, Phlebotomist, or any other healthcare related field
  • Associate and/or bachelor's degree
  • Cultural competence and sensitivity to the needs of a diverse Unaccompanied Minors population.
  • Experience must focus on community-based programs, and/or direct practical experience with UM


Physical Demands:

  • Bending, stooping, and lifting a minimum of 15-25 lbs. required to complete daily tasks
  • Must have the ability to remain in standing position for extended periods of the workday


Work Environment:

Ability to work a flexible work schedule is required dependent upon the department, facility, or program in which s/he works, and job duties assigned. Work schedules are subject to variation from time to time depending on the needs of the department, facility, or program. The work is performed in a shelter setting, which operates 24 hours a day, 7 days a week, 365 days a year. To satisfactorily perform all job duties for this position, medical provider and staff are expected to be available to work at any time as directed by his/her supervisor. Ability to travel in and out State if needed of the time.
