Resident Service Coordinator - Glendale, Arizona Summary: The Resident Services Coordinator (RSC) is responsible for assessing and identifying resident needs focusing on housing success. The RSC will proactively link residents to supportive services and other community resources. NOTE: Due to the various communities' sizes and complexities, this position will also have a dotted-line relationship with the Administrator and/or Resident Services Manager positions to ensure continuity of information flow. Responsibilities:
The duties listed below are essential functions of the position that must be performed to effectively perform the position in at least an acceptable manner as defined by Front Porch. In the future, other duties may be assigned and may also be considered essential functions of the position.
- Provide non-clinical case management and referral services to all resident site (s), which includes securing the full range of social services as needed by individual residents.
- Conduct initial assessment of resident move-in and regular updates.
- Utilize and maintain accurate and timely documentation with agencies.
- Establish rapport with on-site team and submit timely reports.
- Organize and coordinate on-site wellness and health improvement programs and events.
- Conduct outreach and engagement, monitoring services periodically, home visits when needed.
- Collaborate and sustain constructive relationships with residents and their families, service providers & property management staff.
- Attend staff meetings and Resident Services Coordinator on-going training and/or conference.
- Assist residents in identifying and accessing needed services, health resources, and serve as a liaison or advocate for resident when help is needed.
- Establish informal and formal partnerships with health and supportive service agencies in the community to ensure that services are available and delivered in a timely and efficient manner.
- Produce program materials such as periodic, Resident Newsletter.
- Perform all other related duties as assigned.

Knowledge, Skills and Abilities:
- Strong verbal, written and interpersonal communication, listening, facilitation, problem solving, and decision making skills.
- Maintain professional boundaries, including exercising objectivity and confidentiality, while building trusting relationships with residents and staff.
- Basic computer proficiency, including ability to maintain data in an electronic database, communicate via e-mail, use internet and operate word processing software to create program materials.
- Cultural competency in working with diverse populations and individuals from different ethnic and racial backgrounds, including working with non-English speaking adults.
- Self-direct and work independently, but also function well as part of a team.
- Experience building relationships and partnerships with local service providers, community institutions and government agencies.
- Understanding of affordable housing operations and fair housing practices.
- Respect for and comfort with allowing individuals to make their own decisions and prioritize their own care goals.
- Ability to multi-task and establish priorities in a sometimes busy and stressful environment.
Education and Qualifications:
- Bachelor's degree in Social Work, Sociology, Psychology, Gerontology, or related field or equivalent experience.
- Experience with or knowledge of HUD regulatory requirements is a plus.
- Excellent written and oral communication required. Additional ability to speak other languages such as Cantonese, Mandarin, Spanish, or Farsi is a plus. Note: Some properties require fluency in specific languages.
- Basic computer skills including the ability to navigate the internet, use Outlook, create Word and Excel documents, and work with a web-based database.
- Must complete annual online training through the learning management system and offsite in-person training as required.
- AZ Driver's License required-driving to various sites is required

- Must be able to remain in a stationary position 20% of the time
- Must be able to frequently move about inside the facility to access file cabinets, office machinery, etc.
- Must be able to constantly move to position self in order to conduct essential job functions when working with residents and their specific therapy needs
- Must be able to constantly operate a computer and other office productivity equipment, such as a calculator, copy machine, and computer printer to perform the essential functions of the job
- Must frequently move items weighing up to 15 pounds across office or facility for various needs
- Must occasionally ascends/descends stairs to provide essential services
- Must have the ability to succinctly communicate information and ideas so others will understand accurately exchanged information
- Must have the ability to observe details at close range (within a few feet of the observer)
- Must consistently work in an office and community type atmosphere
- Able to drive 30% of the time to residents' apartments throughout the region (mileage reimbursed at current federal rate).
- Able to tolerate possible exposure to fumes, dust and other environmental elements.
- Must be able to work under time demands and remain calm and professional with a wide variety of personalities.
- Simple grasping and fine manipulation, sitting at a desk while using a computer, and using a telephone or extended periods of time.
- Must have the ability to maintain professional boundaries, including exercising objectivity, while building trusting relationships with residents and staff.
