Job Summary
The Senior Director of Healthier Communities is responsible for improving regional community health, community outreach, engagement, and health equity strategies to advance the community oriented strategic direction of the health system. They will provide oversight and lead the strategic direction and operations of community health programs, initiatives, grants, contracts and workplans. The Senior Director of Healthier Communities is responsible for partnering with other health equity regional leaders and the Health Equity Evaluation Analytics and Assessment team (HEEAAT) in the management of community benefit compliance, evaluating community program outcomes and impact provided within the Region. They are responsible for the operational leadership of partnerships, to ensure strong relationships and voices are heard among local and regional strategic partnerships made up of community organizations, government, neighborhoods, and community residents.
Essential Functions
  • Leads the development, execution, and evaluation of community programming. Provides strategic oversight of all operations and strategies. Partners with regional market leaders, Community Boards, and Advisory Councils to support all regional community health initiatives. Works collaboratively with regional leaders, HEEAT and Corewell Health leadership to advance community health programming.
  • Provides leadership and oversight for community health programs, services, and initiatives.
  • Leads Health Equity initiatives across region including relevant councils and committees.
  • Collaborates with clinical service lines and departments to understand and needs of patients in the community and integrate health equity, community health, and SDoH. Supports population health management. Provides guidance, plans, organizes, and directs services, programs, strategic planning, and business development related to community and hospital/physician initiatives. Facilitates procurement and administration of grants and reimbursements to provide quality services. Collaborates with other members of the community / universities / team members to continuously improve services.
  • Facilitates collaboration across members of the Health Equity Council, including opportunities for entities, lines of business and state partners to support programs to advance health equity. Be responsible for strategic plan development, staffing plans, and methodology development for the Health Equity Initiatives function.
  • Serves as a conduit between entities, system community benefit and health equity leaders and data analytics, EPIC, and business intelligence teams to identify appropriate means of measuring impact of key initiatives and publish a system Health Equity Dashboard. Work collaboratively to ensure a regular cadence of reporting to measure progress and support pivots as needed.
  • Delivers quality Community Development services through the consistent development, implementation, management, scaling and evaluation of new and ongoing initiatives, ongoing process improvement activities, and through support of an educational plan to meet the needs of staff and community health professionals.
  • Leads development of required IRS Community Health Needs Assessment and Implementation Strategy process for the seven West hospitals, including development and engagement with leaders in marketing, hospital leaders, government relations, finance, planning, tax, compliance and others. Ensures system-wide strategic alignment and operational excellence in community benefit administration, reporting, and compliance. This includes ensuring completion of community health needs assessment and plan every three years by engaging boards, leaders and external stakeholders in understanding findings and priorities.
  • Leads the Community Commitment Advisory Committee, including partnering with the board chair on facilitating the governance board meetings Ensures strict adherence to the Charter, including all appropriate oversight, budget development and monthly reporting. Sets benchmarks for financial portfolio and ensure targets are met. Oversees and leads grant making.
  • Develops and implements a sustainable funding pathway to supplement the operational budget. Provides leadership for a grants strategy which focuses on external funding for new and existing programs. Ensures compliance with all regulatory requirements as they relate to grants. Partners with SHORE Office of Sponsored Programs to ensure all state/federal obligations are being met for grants.
  • Leads/manages a team. Establishes performance goals, allocation of resources and policies. Hires new staff as needed. Provides orientation, training, and continuing education of employees in area(s) of responsibility.
  • Assures appropriate distribution of community benefit funds within the community / partners of Corewell Health. Provides oversight and approval of the internal community grants process. Engages local and regional stakeholders for input and feedback. Collaborates with urban and rural community organizations, and supports initiatives which align with Corewell Health priorities.
  • Provides leadership to ensure internal programs and services have a defined plan for evaluation and / or a design for research directed toward outcome measurement. Assists with quality reporting and metrics. Provides leadership for all Corewell Health operational deployment system initiatives, including reporting back to Corewell Health leadership when appropriate. Provides support for internal evaluation and scientific research. Assures that research and evaluation efforts are integrated and coordinated with other research and evaluation efforts throughout Corewell Health. Commits resources to external partnership organizations to ensure evaluation and outcome support.
  • Creates new relationships and collaboratively nurtures existing alliances and partnerships with community organizations and local governments to foster optimal community health outcomes.
  • Prepares written and verbal presentations of findings, conclusions, and recommendations to share with executives, Hospital Community Advisory Boards, Board of Director Reports, and in required IRS Form 990 Schedule H tax returns.
  • Performs all operational, financial and administrative management activities pertinent to the role and scope of responsibilities.
  • Serves on internal leadership groups and committees, including Corewell Health representing Healthier Communities.
  • Develops and maintains current knowledge of health equity, community health (rural and urban), federal regulatory requirements related CMA, IRS, and Joint Commission principles and practices. Informs/educates appropriate Corewell Health representatives of current trends.
  • Provides ongoing support in the development of grants in partnership with Corewell Health Foundation team and Office of Research, including evaluation of programs and creation of reports to funders.
  • Develops annual department budget on behalf of Vice President, Community Health Investments.
  • Develops regular stewardship reports for donor-provided funds (i.e. Kosch Grant). Manages relationship and ensures completion of program related activities.
  • Regularly meets and interfaces with Government Affairs team, including VP Government Affairs and Government Affairs Managers to ensure communication and alignment of efforts across all regional communities. Communicates, collaborates, and acts as a community advocate, providing ongoing communication with clients, patients, staff, leadership, quality care issues, and program goals. Participates as an active member on identified hospital / health system / community committees. Advocates for state and federal policy change, as needed.
Qualifications

Required

  • Master's Degree Public Administration, Public Health, Social Work, Business Administration or other related field
  • 10 years of relevant experience, minimum 10 years' experience in a management role with proven track record of effectively leading a performance-based and outcome-based program and staff.
  • Demonstrated experience developing and operationalizing strategies that have taken a program or organization to the next stage of growth.
  • Experience in recruiting, managing, and developing a strong team of staff and program/project managers.
  • Demonstrated experience in grant writing/fundraising experience with the ability to engage a wide range of stakeholders including advisory groups, coalitions, foundations, corporate sponsors and physicians.
