JOB DESCRIPTION: SUMMARY OF POSITION Serves as an integral member of the multi-disciplinary team and a core facet in the continuum of care for Behavior & Primary Healthcare Coordination (BPHC)-enrolled clients. Provides individual, supportive coaching to improve physical health status; coordinates client care with team of internal/external providers; aids clients in accessing/utilizing resources; and promotes the best outcomes for our clients. QUALIFICATIONS Education Level High School diploma or GED required. BA/BS from a four-year college/university preferred. Equivalent work experience may be substituted. Years of Experience Two years of direct service experience working or volunteering in a human service organization.
