Summary:

Reporting to the Manager of Pediatric Clinical Social Work evaluates diagnoses and treats a wide range of psycho-social problems applying individual clinical social work skills in order to assist patients and their families in the resolution of personal and environmental problems attributable to mental or physical illness which prevent them from achieving their highest level of psycho-social functioning.

Responsibilities:

Completes biopsychosocial evaluations.

Provides consultation regarding patient*s background psycho-social functioning and needs to medical and support staff in order to assist in planning a comprehensive care program for the patient.

Develops treatment plans and initiates clinical intervention utilizing individual and group treatment modalities and analyzes course and results of treatment.

Serves a unit based shared team leader role with care management staff as part of the Triad Model to manage patient flow and address psychosocial needs of patients and families.

Participates in collaborative care rounds on all nursing units follows through on clinical social work activities and responsibilities and escalation of cases to complex care rounds.

Participates as a team member in conferences concerning patients with other health care personnel and community agencies.

Assists patients and families in understanding accepting and following medical and social work recommendations designed to restore the patient to optimum functioning within their capacity.

Assesses patient and family advocacy needs and coordinates appropriate advocacy activities within the hospital and in the community with an emphasis on self-determination for the client system.

Provides field placement supervision and evaluation for BSW and/or MSW degree candidates as part of the Department of Clinical Social Work Student program at the request of the Manager of Pediatric Clinical Social Work.

Assist with department orientation of new social work staff as needed.

In accordance with departmental standards regarding frequency and content document social work activity in patients* medical records.

Completes case related screeners at the request of management and participates in relevant grant opportunities.

Participates in departmental and community studies and projects as assigned.

As relevant to clinical assignment remains current on available community resources and accesses them as needed.

Maintains and enhances professional expertise through appropriate educational activities. Attend training for specific areas of clinical responsibility and to maintain licensure

Participates in quality assurance activities in accordance with department hospital and The Joint Commission standards.

Adheres to departmental and hospital policies and procedures to maintain a safe environment of care.

Collaborates with clinical staff throughout the system provides coverage as needed due to high volume and acuity and participates in a multi-disciplinary team with a goal of quality patient care and efficient patient flow.

Other duties/projects as assigned by the Manager of Pediatric Clinical Social Work.

Other information:

BASIC KNOWLEDGE:

Master*s degree in Social Work and licensed through the RI Department of Health at a minimum BUHCSW level.

Upon eligibility it is mandatory that they pursue LICSW. status.

Training and experience necessary to address the specific needs of the patients served in assigned area of responsibility; this includes issues relating to age and stage of development and their special needs and cultural patterns of the patient populations being served.

Analytical and therapy skills necessary to evaluate patients develop treatment plans initiate therapy and evaluate results. Ability to effectively interact with patients families medical staff and outside agencies in providing services.

EXPERIENCE:

Between six months and one year of clinical experience preferably in a health or mental

health setting with an emphasis on populations to be served in the assigned area.

Experience with Crisis Stabilization and Suicide Assessment preferred.

INDEPENDENT ACTION:

Performs independently within department policies and practices. Refers specific complex problems to manager where clarification of departmental policies and procedures may be required.

SUPERVISORY RESPONSIBILITY:

May provide supervision to MSW students.
