Summary:

The Advanced Practice Provider (APP) reports to the Vice President of the Center for Lung Health and the Center for Lung Health Director. The APP works as an integral member of the Center of Lung Health (CLH) with pulmonologists interventional cardiologists cardiothoracic surgeons pulmonary and cardiology fellows advanced practice providers nurses echocardiography technicians pharmacists and medical assistants. The APP is an integral part of the Center for Lung Health team and provides care across the continuum from consult to post-procedure care for patients within the ambulatory and inpatient settings. The APP will be a part of the program that will provide destination care for patients with advanced pulmonary and cardiopulmonary diseases including all forms of pulmonary vascular disease thromboembolic disease and interstitial lung disease.

Responsibilities:

The expectation of the APP in this role is for autonomous management of pre- and post-operative care. Coordinates the daily operational activities required for this program and communicates the plan with the care team. Responsibilities will include but are not limited to navigating the patient through the referral process from home outside hospitals and the inpatient setting by performing history and physicals expediting workups and collaborating with the care team to efficiently drive patients through the system. Triages patients to the appropriate care resource (includes direct contact and telephone encounters).

The APP may be responsible for completing pre-admission testing sick visits outpatient calls and discharge planning and post-operative follow up clinic visits and collaborates with physician colleagues.

Works with Pulmonary Cardiology and Brown University Health leadership to develop implement and track relevant quality outcome and patient satisfaction measures. Oversees ongoing quality assurance activities by collecting measuring and analyzing statistical data collects and reports outcomes (internal/external) including but not limited to length of stay patient satisfaction operational and functional measures and others as requested.

The APP will work with our clinical and translational research team to support the advancement of pulmonary and cardiopulmonary research within the organization.

Collaborates as a skilled communicator working among multiple departments simultaneously. Demonstrates ability to be highly organized with foresight for prioritization and action. Ensures compliance and achievement of Hospital wide Quality Improvement Plan. Assists with community outreach and awareness efforts.

May perform other duties as requested by staff physicians or attending physicians including care of selected patients on both the medical and surgical services and initiating consultative services on behalf of the Center for Lung Health.

Attends weekly team meetings and M & M*s and Pulmonary Critical Care and Sleep divisional conferences as appropriate.

Participates in the education clinical research and mentoring of healthcare providers. Assists in the development and maintenance of Brown University Health clinical polices that are pertinent to the care of patients in the Center for Lung Health.

Performs other duties as assigned.

BASIC KNOWLEDGE:

Physician Assistants must be graduates of an accredited PA program with current NCCPA certification.

Nurse Practitioners must be graduates of a Nurse Practitioner program and certified by the ANCC or by the AANP.

Candidates should hold or be eligible for licensure in the state of Rhode Island as a Physician Assistant or Nurse Practitioner eligible for licensure for Federal DEA certificate. Completed approved BLS/ACLS courses.

EXPERIENCE:

Ideal candidates would have 1-2 years pulmonary cardiology or cardiac surgery experience but highly qualified new graduates will be considered. On-the-job training and experience to become familiar with job responsibilities and work routines will be provided. All applicants should possess a high level of clinical autonomy.
