Job Summary
The Medicare Clinical Pharmacist will work with the Medicare Quality team to support Priority Health's Medicare Stars initiatives. The candidate for this role will telephonically engage Medicare members to complete comprehensive medication reviews for Medication Therapy Management (MTM), assist with improving adherence to cholesterol, diabetic, and high blood pressure medications, and support Star measures.
Essential Functions
Deliver a patient-centered comprehensive medication review covering prescription and non-prescription medications including herbals and supplements to identify and resolve medication related concerns Evaluate medication appropriateness, effectiveness, and patient safety utilizing evidence-based medicine and national guidelines to educate the patient and/or advise the provider to ensure the medications are meeting the patient's needs and goals Identify and provide clinical recommendations to patients, caregivers, and/or providers that address gaps in care, high risk medication use, drug interactions, adverse drug events, medication adherence, supra or suboptimal medication dose, untreated health problems, and other clinical concerns Educate patients on the purpose of their medications, goals of therapy, proper administration, monitoring, and medication adherence to achieve optimal outcomes Assist in managing medication cost concerns by identifying cost effective interventions such as dose optimization, formulary alternatives, therapeutic interchanges, etc. Collaborate with patients, caregivers, and providers to develop a care plan with individualized goals and personalized interventions that address concerns identified through the consultation that lead to better outcomes Utilize motivational interviewing to identify and address barriers to adherence Directly support Medicare Quality initiatives that improve Star ratings for, but not limited to, medication adherence to cholesterol/blood pressure/diabetic drugs, Statin Use in Persons with Diabetes (SUPD), Statin Use in Cardiovascular Disease (SPC), Medication Therapy Management (MTM), and CAHPS Educate patients on their Medicare Part D benefits and drug coverage to increase understanding and improve member experience Coordinate with internal and external partners such as pharmacies, providers, vendors, etc. to improve health outcomes Maintain a positive and professional attitude when interacting with patients, providers, internal teams, and vendors Keep current with new and emerging clinical trends Identify and recommend operational efficiencies Perform other duties assigned by the Manager of Medicare Quality of Director of Medicare Pharmacy
Qualifications
Required
- Doctorate Doctor of Pharmacy degree from an accredited College of Pharmacy
- LIC-Clinical Pharmacist - STATE_MI State of Michigan Upon Hire required Or
- LIC-Clinical Pharmacist - STATE_OTHR State Other than Michigan Upon Hire
Preferred
- Two (2) + years of work experience as a licensed pharmacist working in retail, providers office, managed care, or PGY-1 residency equivalent
- Active and unrestricted pharmacy license in the State of Michigan
- Experience with Medicare Stars and quality measures
- Certified Geriatric Pharmacist or BPS certified
- MTM Experience with MTM certificate and/or BCMTM
- Proficient in Microsoft Office such as Outlook, Excel, Word, Power Point
- Ability to work independently and as part of a team
- Comfortable with change
- Excellent written and verbal communication and relationship building
- Ability to exercise strong clinical judgment and interpersonal skills
- Ability to manage priorities and deadlines
