Full Time, 1st Shift

Description:

• Serves as a clinical resource, patient advocate, and system navigator for cancer care and serves as a liaison between the organization, patients, families, and the community for cancer groups
• Provides written and verbal breast cancer information related to the patient's diagnosis, pathology plan of treatment, and survival plan of care
• Facilitates information gathering and analysis for patients to make informed decisions
• Assesses patient's educational and physio/psychosocial needs and barriers to care
• Individualizes the care of breast cancer patients throughout the continuum of care
• Collaborates with multidisciplinary team members to facilitate timely access and continuity of care across various care settings
• Works with healthcare providers in maintaining concordance with evidenced-based care and with established and adopted guidelines
• Enhances self-development on a continual basis by maintaining current knowledge in clinical education, reviewing professional literature and participating in professional organizations

Requirements:

• Bachelor of Science in Nursing (BSN)
• Minimum of two years of relevant work experience
• Current KY state license as a registered nurse
• Basic life support
• Oncology nursing experience preferred but not required
• Must obtain Certification in Oncology Nursing (OCN, AOCN, AOCNS or AOCNP) within 1 year of hire
