Job Description: Nurse Navigator Lexington, KY Full Time, 1st Shift Baptist Health outpatient oncology, where compassionate care meets advanced treatment. Our cancer center offers medical oncology, infusion therapy, radiation oncology, and genetic counseling. We also provide nutritional counseling, palliative care, psychological support, and access to support services. - Serves as a clinical resource, patient advocate, and system navigator for cancer care and serves as a liaison between the organization, patients, families, and the community for cancer groups
- Provides written and verbal breast cancer information related to the patient's diagnosis, pathology plan of treatment, and survival plan of care
- Facilitates information gathering and analysis for patients to make informed decisions
- Assesses patient's educational and physio/psychosocial needs and barriers to care
- Individualizes the care of breast cancer patients throughout the continuum of care
- Collaborates with multidisciplinary team members to facilitate timely access and continuity of care across various care settings
- Works with healthcare providers in maintaining concordance with evidenced-based care and with established and adopted guidelines
- Enhances self-development on a continual basis by maintaining current knowledge in clinical education, reviewing professional literature and participating in professional organizations
Requirements:
- Bachelor of Science in Nursing (BSN)
- Minimum of two years of relevant work experience
- Current KY state license as a registered nurse
- Basic life support
- Oncology nursing experience preferred but not required
- Must obtain Certification in Oncology Nursing (OCN, AOCN, AOCNS or AOCNP) within 1 year of hire
