Description Job title Case Manager /Intensive Case Manager Reports to Program Director Position Summary The case manager will determine resources needed to meet the individualized needs of the participant. The case manager will take all necessary actions in assuring the implementation of the service treatment plan including serving as an advocate for the participant to enable him/her to receive needed services; identification of program needs, service gaps and recommending strategies. Essential Duties Addendum FASST Connections (titled Intensive Case Managers per funder). Staff are required to ensure 24-hour access to Intensive Case Management services.
