Position Summary |
The primary responsibility of the Medication Access Program (
MAP) Clinical Pharmacist is to serve as a liaison between the transplant patient and the
MAP staff and assist the
MAP staff with quality control. Additionally, the
MAP Clinical Pharmacist is responsible for providing clinical pharmacy services to kidney or kidney/pancreas transplant patients in the Augusta University transplant clinic. The
MAP Clinical Pharmacist shall also be a medication resource for patients and the medical staff at Augusta University Transplant Program. |
Knowledge, Skills, Abilities and/or Competencies |
Ability to establish and maintain effective working relations with physicians, social workers, transplant coordinators, patients, and other healthcare professionals. Knowledge of computerized information systems, and
HIPAA privacy policies. |

- Assist the MAP staff in coordinating with the patient through the enrollment process
- Assist the MAP staff with conducting quarterly audits
- Work with social worker(s) to help identify patients who may need the services of MAP
- Serve in transplant-related research to the Director of MAP

- Document the patient's medication profile in the electronic profile
- Identify interactions and make recommendations to physicians
- Identify problems with the patient's medication regimen such as compliance/adherence
- Make recommendations for dosage changes or medication regimen changes to physicians

Drug Resource Duties
- Prepare medication administration guides for patients to assist them in knowing what their medications are for and when to take them
- Provide drug information for physicians and medical staff
- Call in or e-scribe prescriptions ordered by physician if needed and when time permits
- Assist Transplant nursing staff with providing the correct information needed for prior authorizations
