GENERAL SUMMARY: Provides the vision, strategic direction and leadership for Bryan Health Connect's (BHC) population and health and value-based care (VBC) operations. Primarily accountable for meeting and improving BHC's value-based contract performance as well as building, maintaining, and implementing key improvement initiatives focused on improving quality, reducing costs, and improving health equity for our members and their patients. Leads a team responsible for working with BHC membership to maintain and improve contract performance and population health infrastructure. Builds and leverages positive working relationships with leaders across numerous partner organizations to successfully implement population health initiatives. PRINCIPAL JOB FUNCTIONS: 1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values. 2. *Collaborates with the President to develop, implement and maintain the population health management strategy to improve care quality and consumer engagement. 3. *Develops and executes operational improvement initiatives with measurable goals. 4. *Evaluates, develops and prioritizes opportunities based on objective criteria (ROI, metric improvement, etc.). 5. *Develops and maintains strong partnerships with key internal and external stakeholders to manage value- based incentive design and implementation for BHC. 6. Leads the data and analytics infrastructure supporting population health operations. 7. *Collaborates with the Vice President of Medical Affairs to develop, communicate and articulate the population health management vision to key stakeholders including physicians and other healthcare professionals, administrators and customers. 8. *Provides strategic leadership to the population health operations team members during the design, implementation, evaluation and ongoing practice of outcomes improvement. 9. Prepares and provides regular, timely and relevant updates to BHC leaders with respect to population health operations, clinical support operations, milestones, ongoing/continuing outcomes and associated expense savings. 10. *Supports and provides direction to the data and analytics team to produce aggregate practice reports and identify opportunities for performance improvement. 11. *Utilizes data and evidence-based research to regularly identify and assess practice variability and define targeted and scalable initiatives for review by the BHC Clinical Guidelines/Quality Assurance Committee and BHC leadership. 12. *Directs and leads BHC VBC programs, initiatives and projects to establish clinical integration priorities to achieve a culture of evidence-based quality care. 13. Develops and maintains strong relationships with representatives of appropriate committees in BHC to establish and agree upon criteria, protocols, care pathways, etc. to meet the goal of providing evidence-based care. 14. *Collaborates with BHC's Clinical Guidelines/Quality Assurance Committee to prioritize initiatives that drive performance in value-based contracts. 15. Continuously monitors the goals and objectives of the population health initiatives while maintaining alignment with the strategic goals and objectives of Bryan Health. 16. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise. 17. Participates in meetings, committees and department projects as assigned. 18. Performs other related projects and duties as assigned. REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: 1. Knowledge of program development and coordination. 2. Knowledge of clinical databases and data management. 3. Knowledge of the U.S. healthcare system, health administration/policy, and the factors that influence healthcare spending, care quality and patient satisfaction across populations. 4. Knowledge of effective models to improve care while decreasing costs. 5. Knowledge of computer hardware equipment and software applications relevant to work functions. 6. Skill in interpersonal communication, written and verbal, for communication on all levels, with the ability to speak effectively to small and large groups of physicians, administrators and clients. 8. EDUCATION AND EXPERIENCE: Bachelor's degree in nursing or healthcare related field required. Master's degree in business administration or healthcare related field preferred. Minimum seven (7) years healthcare experience required, which includes direct experience with clinical integration, population health, health care administration, policy or value-based programs. Prior leadership experience in clinical integration, population health, accountable care organization (ACO), VBC and/or disease management required. PHYSICAL REQUIREMENTS: (Physical Requirements are based on federal criteria and assigned by Human Resources upon review of the Principal Job Functions.) (DOT) Characterized as sedentary work requiring exertion up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, pull, or otherwise move objects.
