Job Title: Rehabilitation Team Coordinator Position Summary: -Works under the direct supervision of the Rehabilitation Therapy Services Team Leader and/or the leadership of the Inpatient or Outpatient Manager - Uses objective evaluation techniques to assess the physical limitations and medical condition of patients - Prescribes and implements a treatment program which is designed to return patients to their maximum level of function - Develops the patient care service to meet the standards of care - Works collaboratively with all members of the multidisciplinary health care team to maximize patient and team outcomes - Promotes improvement in service, department and hospital systems to benefit patient care - Incorporates and operationalizes the goals and objectives of the Hospital at the site of care - Mentors others to promote quality and excellence of care - Coordinate patient care activities on the service Education & Qualifications:

Salary Range: Minimum 41.09 per hour - Maximum 67.84 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
