We are seeking compassionate and dedicated Hospice Counselors to join our team. This role is ideal for individuals with a Master's or Bachelor's degree in Social Work, a Licensed Vocational Nurse (LVN) background, or Chaplain experience. As a Hospice Counselor, you will play a critical role in providing emotional and practical support to patients and their families during an important time in their lives.

Your responsibilities will include building meaningful, supportive relationships with patients and referral sources, addressing patient inquiries, and explaining key details about hospice services. You will also assist in obtaining necessary consents and help families understand their benefits. • Establishes and maintains positive working relationships with current and potential referral and payer sources
• Builds and monitors community, customer, and patient perceptions

• Occasional on-call responsibility as appropriate for discipline
• Ensures compliance with all state and federal legal and regulatory requirements
• Additional responsibilities as assigned

• Master's degree in Social Work, Bachelor's degree in Social Work, Licensed Vocational Nurse or Chaplain
• One year experience in health care marketing, preferably in hospice care
• Ability to deal tactfully with customers and the community
• Knowledge of corporate business management, governmental regulations, and Joint Commission standards
• Demonstration of good communication and public relations skills
• Demonstration of autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities
