Who we are:

Henry Ford St John Children's Hospital is a nationally recognized children's hospital located within Henry Ford St. John Hospital. The approximately 90 bed hospital serves all pediatric populations from birth to 21 years of age, including an 8 bed Pediatric Intensive Care Unit, 26 private-bed inpatient pediatric unit and a level II pediatric emergency department.

GENERAL SUMMARY:

As the pediatric/PICU Clinical Pharmacy Specialist at Henry Ford St John Children's Hospital you will be an integrated pharmacy resource within the pediatric unit and PICU and will be actively sought out by the medical and multidisciplinary teams for your expert medication knowledge. You will play an essential role in optimizing our pediatric patients' medication management through daily patient care activities and through policy, protocol and order set development, as well as ongoing drug use evaluation and safety/quality initiatives. You will also have the opportunity to participate in the mentoring and education of Pharmacy Residents, Doctor of Pharmacy students and other health care professionals. In addition, through our patient centered care model you will provide medication education to our pediatric patients/parents/families/caregivers.

Our team of decentralized patient care pharmacists all practice as clinical pharmacists and cross cover the PICU and pediatric unit on afternoons and weekends. In collaboration with our NICU Pharmacy Specialist, you will play a vital role in helping maintain and advance our clinical pharmacists neonatal/pediatric medication knowledge.

PRINCIPLE DUTIES & RESPONSBILITIES:



  • Reviews and assesses medication orders to identify, prevent and resolve potential and actual drug related problems.
  • Recommends formulary alternatives when appropriate.
  • Compounds and dispenses medications for accurate delivery to patients. Provides drug information and education to medical staff and patient/families/caregivers.
  • Communicates with physicians and other clinicians as necessary to facilitate care
  • Oversees technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies.
  • Assists with academic functions such as serving as a preceptor for pharmacy students.
  • May float to provide coverage of pharmacy needs

CERTIFICATIONS/LICENSURES REQUIRED:



  • Pharmacist credentialed from the Michigan Board of Pharmacy obtained prior to hire date or job transfer date required.


EDUCATION/EXPERIENCE REQUIRED:



  • Bachelor's or advanced degree in Pharmacy required
  • Post-graduate year 1 pharmacy residency required
  • Post-graduate year 2 pharmacy residency in pediatrics highly preferred
  • Pediatric advanced life support certification preferred.
  • Expectation is that Pediatric/PICU pharmacist specialist will be Board Certified for Pediatrics within 1 year of hire
