Summary:

Under the supervision of the Clinical Supervisor and the Director Access and abiding by all applicable policies and procedures engages families during and/or following an ED visit for behavioral health and provides care coordination to ensure patients receive appropriate follow-up care.

Responsibilities:

Provides care coordination to youth and families during and after an ED visit for behavioral health to include:

  • Engage youth and families during an ED visit to assess aftercare needs
  • In collaboration with patient and family complete an aftercare plan which will provide the patient and family with appropriate behavioral health services
  • Identify coordinate and schedule behavioral health follow up appointment in collaboration with family/child within seven days of ED visit or as soon as possible given provider availability.
  • Assist family with accessing appropriate behavioral health services as outline in aftercare plan
  • Maintain at minimum weekly telephonic contact with family/child to support family/child in following the agreed upon after care plan.
  • Inform and collaborate with child*s Pediatrician on engagement in program and aftercare plan.
  • Collaborate with and refer to community organizations to help families overcome barriers to treatment such as homelessness rides to and from appointments and trouble obtaining medication.

Maintains knowledge of current behavioral health programs and services available to children in Rhode Island.

Checks in with families two months after initial appointment kept to determine if child is still in care. Provide additional resources and referrals if needed.

Develops and maintains a database to monitor program outcomes such as: number of patients seen in ED for behavioral health concern who are discharged; number and percentage of patients/families who engage with a care coordinator; seven-day follow-up appointments scheduled and number and percentage of appointments kept; number of contacts per family; and any other data needed to assess the effectiveness of the program.

Completes documentation within LifeChart according to established policies and procedures.

Attends individual supervision with the Clinical Supervisor of the Access Center.

Participates in program related meetings including but not limited to staff meetings clinical conferences and in-services.

EDUCATION:

BSW from an accredited school of Social Work. A bachelor*s degree in a related field may be considered when accompanied by at least one year of relevant experience in case management or care coordination.

EXPERIENCE:

Relevant case management experience required.

Demonstrated ability to interact professionally both on the phone and with colleagues.
