We are seeking a Mobile Case Manager to join our Mobile Methadone Clinic team in Philadelphia, PA. This posting is for full-time, first shift, Monday - Friday 7:00am - 3:30pm. Earn $22.70/hr.! As part of the Mobile team, the candidate must be willing to work within both the Brick and Mortar and Mobile RV environments. We are a Mobile Methadone Clinic for high-risk adults in need of same-day care that is not an emergency in Philadelphia. We are 100% community-based conducting mobile engagement and the first of its kind in the City of Philadelphia! We serve individuals with substance use concerns by providing Mobile Methadone Maintenance Treatment (MMT) and Medication Assisted Treatment (MAT). The Case Manager will serve as an advocate and motivator, providing a range of person-centered community-based supports and services. Responsibilities include but are not limited to the following. * Assist in assessing each client's individualized needs and coordinating treatment services with local hospitals, rehabilitation centers, and throughout all Merakey Parkside Recovery sites/programs. * Take the lead in managing each participant's transportation needs. * Serve as a core member of a collaborative care team, assisting in rapidly connecting patients with OUD to medically assisted treatment and navigating the appropriate follow-up. * Assist with the intake process as well as provide linkage to other programs and coordinate warm hand-offs to support patient engagement. The ideal candidate will possess the following qualifications: * At least 2 years of experience working with a specialty in Opioid Use Disorder (OUD)/Substance Use Disorder (SUD) and minimum 6 months case management experience is required. * High school diploma or equivalent is required. * At least 18 college credits toward degree in Behavioral and Human Services is required. * American Society of Addiction Medicine (ASAM) certification is required. * Knowledge of Doximity, Microsoft Excel, Microsoft OneDrive, Microsoft Teams, and Zoom is required. * Must be willing and able to work remotely from all Merakey Parkside Recovery sites with intake, clinical, and medical teams. * Must possess knowledge of the intake/assessment processes and guidelines within Medication Assisted Treatment (MAT) (specifically Methadone Maintenance Treatment - MMT) programming. * Strong knowledge of local behavioral health and supportive services. * Strong knowledge of benefits coordination (specifically Medicaid and Medicare). * Experience working or volunteering within the community providing direct support services to unhoused individuals that use substances. * Empathy, compassion, and non-judgmental attitude. * Ability to work collaboratively in a multi-disciplinary team.
