Overview

  • Location: MercyCare Building, Janesville WI
  • Full Time: 80 Hours/2 Weeks
  • Schedule: Monday - Friday, Day Shift, Flexible Hrs available

The Health and Wellness Educator is responsible for the implementation of assigned program aspects that address the objectives of the health plan and the goals of our members. The individual will be able to apply program evaluation and accepted clinical research findings to secure clinical and program outcomes. The individual will build strong member relationships, actively explore member needs, and pursue creative solutions to stratify those needs and monitor results on a continual basis. The Health and Wellness Educator will assist in monitoring quality initiatives and reviews, including but not limited to, focus studies, clinical guidelines and preventative health guidelines. The Health and Wellness Educator will attend, contribute, and present as required to internal committees and to employer groups.


Responsibilities

Essential Duties and Responsibilities

  • Provide education to individuals and families on a wide variety of topics, with an emphasis on education for diabetes, cardiovascular conditions, preventative healthcare, and additional chronic conditions in alignment with current quality measures.
  • Facilitate member education meetings, webinars, communication materials, and on-site health events.
  • Schedule members for regular status calls, calendar planning, and result reviews.
  • Determines specific objectives, goals, and actions designed to meet the members' needs as identified through the assessment process.
  • Help members understand their benefits/coverage and navigate the health system
  • Establishes goals and strategies for member centric coordination across settings with the member, family, community resources, and/or other care providers.
  • Organizes, integrates, and modifies the educational resources necessary to accomplish the goals and establish a plan for a healthy living.
  • Monitors the members' ongoing process towards healthy living and its activities to determine the plan's effectiveness.
  • Participates as educator and resource person for provider offices and hospital staff as it pertains to the health plan policy and procedures.
  • Demonstrates knowledge of NCQA Accreditation Standards and HEDIS Standards and participates in initiatives to achieve and maintain accreditation and improve HEDIS compliance rates.
  • Assists in the gathering and analysis of data.
  • Insures that all activities are internal and external customer focused, well planned, organized, and controlled to assure the cost effectiveness of all programs.
  • Serve as a liaison between key clinical and community partners instrumental in implementing educational programs.
  • Support the creation of monthly health education newsletters for distribution.
  • Develop monthly health-related articles for internal and external promotion.
  • Develop educational materials for patient education.
  • Participate in relevant team meetings.
  • Create and present onsite health presentations to employer groups.

Education and Experience

Bachelor's degree preferred or Associate Degree and two years related experience in healthcare.
(RN, LPN, Life Coach, Social Worker)

Certification and Licensure

Certified Health Education Specialist (CHES) certification
Certified Dietitian or equivalent preferred.

Skills and Abilities
Demonstrates knowledge of NCQA Accreditation Standards and HEDIS measures.
Provides education to members who meet specific criteria for wellness programs.
Demonstrates ability to work within a team environment
To perform this job successfully, an individual should have knowledge of the Microsoft Office Suite, including Excel, PowerPoint, Word, and Outlook.
