Monday-Friday 8a-4:30p at the EMS building, Downtown Atlanta.

Summary:

The Population Health Specialist will be responsible for managing operational and administrative processes for Mobile Integrated Health (MIH) department to reduce readmissions of recently discharged patients with chronic issues. The Medical Assistant interfaces with and acts as a resource to team members, other Grady Health System departments, patients/caregivers and physicians/care providers, and external partners including home health agencies, transportation vendors, durable medical equipment/medical supplies providers and community service organizations.

Qualifications:

* High School Diploma or GED.

* Medical assistant certification (preferably CCMA).

* Three or more years of experience in a health care or community resources setting.

* Experience with EMR and computerized charting, preferably in Epic EMR.

* At least 2 years of college course work preferred.

* Knowledge of medical terminology, Microsoft Office, especially Excel.

* Case management experience preferred.

* Excellent customer service skills.

* Local knowledge about and connections to community health care and social resources

* Ability to multitask and organize.

* Ability to speak a second language, preferably Spanish.
