Family Health Centers of San Diego is hiring for an experienced Physician, Nurse Practitioner or Physician Assistant to join our PACE team!

Details/Requirements:

  • Full Time (May consider part time at a minimum of 32 hours per week)
  • Experience working with older adults required
  • Bilingual Spanish, Farsi or Arabic preferred
  • For NP and PA applicants: Must have a minimum of 2+ years' experience working in an out-patient clinic setting similar to a PACE model which includes providing care to a panel of older adults.

Job Roles

  • Provide primary care and acute illness evaluation and treatment for PACE participants.
  • Perform comprehensive assessments for participants and coordinate with the interdisciplinary team (IDT) to develop a comprehensive care plan for each participant.
  • Be an active member of the IDT communicating participant changes, collaborating on care planning decisions and coordinating care delivery.
  • Work collaboratively with network providers and other members of the IDT to ensure effective care coordination, particularly around hospital discharge, short-term and long-term placement.
  • Oversee and manage the PACE participant's use of medical specialist and inpatient care.
  • Manage all care of PACE participants' in the nursing home providing regular visits as dictated by PACE's standards and participants' needs.
  • Review outcomes and utilization data, to include adverse events, hospitalizations, and use of specialists and collaboration with IDT to identify opportunities for improved care and operations.
  • Maintain appropriate medical records and related documentation to include licensure and board certifications.
  • Participate in on-call schedule in a responsive and timely manner.
  • Perform other duties as assigned.

Education/Certifications/Licenses/Registrations

  • Graduate from an accredited school of medicine.
  • Must possess current unrestricted license to practice in the State of California.
  • Board certified in Internal Medicine or Family Medicine, with at least one year of training or experience in Geriatric Medicine, or Board Certified in Geriatric Medicine, required.
  • Current American Heart Association healthcare provider, CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training.
  • DEA license.
  • Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Age 21; Licensed for a minimum of 3 years; No violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving within 7 years.

Experience/Specialized skills (including Language)

  • Experience and comfort with older adults.
  • Ability to work effectively with PACE participants from diverse social, cultural and economic groups.
  • Comfort managing a full range of chronic and acute conditions.
  • Experience working within an interdisciplinary team environment.
  • Knowledge and comfort working in a managed care setting.
  • Ability to practice medicine in accordance with appropriate PACE guidelines.
  • Cultural and linguistic sensitivity to populations served required.
  • Basic computer literacy ability to comply with record keeping needs (i.e., E-HR documentation).

Note: The compensation range advertised is for a Physician candidate. Compensation for an NP/PA is: $135,000 - $165,000 based on years of experience.

In the spirit of pay transparency, we are excited to share the base range for this position, exclusive of fringe benefits.

$250,000.00 - $275,000.00

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
