Job Description:

  • The Clinical Pharmacist is responsible for providing clinical judgment, professional knowledge, and distribution skills in the Pharmacy Department to ensure that patients receive pharmaceutical care in alignment with the goals of the Department and the Organization. The role includes reviewing drug orders, patient profiles, and medical records to assess the appropriateness of drug therapy.


Role & Responsibilities:

  • Review drug orders and patient profiles to evaluate the appropriateness of drug therapy.
  • Select, compound, prepare, and dispense drugs in accordance with departmental procedures, as well as all applicable laws and regulations.
  • Oversee and direct the workflow of technicians and interns, ensuring efficiency within the department.
  • Consult with medical staff, nursing staff, patients, and other hospital personnel regarding drug therapy, serving as a key resource for drug information.
  • Maintain accurate and complete patient profiles following departmental procedures.
  • Perform routine pharmacy profile-nursing medication administration record (MAR) comparisons.
  • Document clinical interventions, formulary compliance, and non-compliance within the Electronic Health Record.
  • Identify and report adverse drug reactions (ADRs) and medication errors as per departmental policies.
  • Under approved medical staff protocols, initiate and/or adjust drug dosages, intervals, routes of administration, or laboratory tests as necessary.
  • Assist with quality improvement projects and data collection as assigned.


Skills & Experience:

  • Experience: At least one year of hospital-based experience in unit-dose distribution and IV additives or completion of a hospital residency is required.
  • Skills:
  • Excellent verbal and written communication skills, particularly in drug information and therapeutics.
  • Ability to apply professional knowledge and judgment in providing pharmaceutical care to patients across all age groups: Geriatric, Adult, Pediatric, and Neonatal.


Interpersonal Skills:

  • Physical Effort:
  • Standing for up to 80% of the shift and lifting up to 25 pounds.


Education & Licensure:

  • Education: Doctor of Pharmacy (Pharm.D.) or Bachelor of Science Degree in Pharmacy with evidence of required continuing education.
  • Licensure: California Board of Pharmacy licensure.
