Overview About Summit ElderCare: Fallon Health operates one of the largest Programs of All-Inclusive Care for the Elderly (PACE) in the country. Called Summit ElderCare, Fallon's PACE program helps older adults and their caregivers to avoid or delay nursing home care. Participants in Summit ElderCare are cared for by a team of professionals and have access to social support and a wide array of medical services in one convenient location while they keep the independence of living in their own homes and communities. Brief Summary of Purpose: The Clinical Supervisor is responsible for oversight of daily operations related to the medical clinical and ADHC. Possesses nursing assessment skills, ability to work within an interdisciplinary team model and knowledge of the nursing care planning process. Excellent interpersonal communication and problem-solving skills. IV skills recommended but not essential. Functions independently in the day-to-day management of the clinical needs of participants and the operations of the PACE center clinic utilizing Fallon Health and Summit ElderCare policies and procedures, leadership guidance and program goals in the decision-making process. Responsibilities Job Responsibilities: * Responsible for recruitment and training of staff within the discipline including Medical Assistants and Health Aides. Qualifications Education: Graduate of an accredited school of nursing License/Certifications: Licensed as an RN in Massachusetts Experience: At least two years of supervisory experience working with frail elders in a health care setting. Supervisory experience with medical assistants and health aides is preferred.
