Summary:
The Stroke and Heart Failure Program Coordinator is responsible for planning, organizing, and directing the comprehensive Stroke Program and the Heart Failure Program. In addition, the Stroke and Heart Failure Program Coordinator will provide clinical support, education, and instruction to the staff and community. The Coordinator will collect and analyze data required to meet metrics and assist in marketing the programs.

Responsibilities:
The Coordinator partners with hospital employees, physicians, other external healthcare agency members; to include the patients and their families/caregivers in the internal and external setting. The coordinator works with key physician groups as well as community organizations including skilled nursing facilities and home health agencies; displaying professionalism in all interpersonal relationships in a manner that supports a positive image of Lenoir Memorial Hospital.

EDUCATION REQUIREMENTS

  • Current licensure to practice as a Registered Nurse in the State of North Carolina. BSN Required.

PROFESSIONAL EXPERIENCE REQUIREMENT
  • Minimum of 5 years of acute care nursing experience; experience in critical care, ED, complex heart failure (HF, COPD), or neurology preferred. Demonstrated leadership/supervision within interdisciplinary healthcare team in chronic illness management is necessary for this role. Excellent computer skills, use of Excel spreadsheets data collection, entry, and analysis. Project management experience beneficial.

LICENSURE/REGISTRATION/CERTIFICATION
  • Current BLS certification. ASLS Instructor and NIHSS certification to be obtained within one year.
