Hours:Monday-Friday 8am - 430pm, Dayshift. NO NIGHTS, HOLIDAYS or WEEKENDS! Overview As anIntegrated Care Managerwith Sentara, you get to work together in an intensely collaborative way with the patients, family, and members of the medical team to provide patients with the resources they need and help prevent them from readmitting into the hospital. This position's primary duty is customizing what is best for the patients. That would include helping with things like home health, long-term care placement, home medical equipment, homeless and community resources. The base pay rate for Full Time employment is:$76,731.20-$127,878.40Additional compensation may be available for this role such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
