Job Summary:
The HBPC Primary Care Physician provides comprehensive, primary medical care services, with an emphasis on optimizing functional status and quality of life, for patients who are not best served in a traditional office setting. The ideal candidate will have a passion for providing comprehensive health care to homebound populations in the context of their social support systems, physical environments, and personal values. The HBPC Primary Care Physician role is a 0.6 FTE time commitment and can be combined with other positions/responsibilities for up to 1.0 FTE role in partnership with the Department of Family Medicine & Community Health, Division of Geriatrics and Gerontology, or Division of General Internal Medicine. Responsibilities: - Provide comprehensive, interdisciplinary, primary care in the homes of older adults with serious medical, social, and/or behavioral conditions for whom routine clinic-based care is not effective or may not be possible. Education: Required Qualifications: -Minimum 2 years work experience providing primary care to older adults or completion of accredited training program in geriatric medicine or hospice and palliative care by start date.
