Qualifications you'll bring:



  • Bachelor's degree in a related field (e.g., nursing, social work, healthcare administration) or equivalent experience.
  • Proven experience in case/care & disease management, preferably in a leadership role.
  • Strong organizational and time management skills to effectively prioritize and manage multiple cases and tasks.
  • Excellent communication and interpersonal skills to build relationships with clients, their families, and healthcare professionals.
  • Ability to analyze complex information and make informed decisions.
  • Proficient in using computer systems and software for documentation and data management.
  • Knowledge of relevant regulations and standards in case management.
  • Compassionate and empathetic approach to working with individuals in need.
  • Curiosity to foster innovation and pave the way for growth
  • Humility to play as a team

Your key responsibilities:




  • Lead a team of case managers and provide guidance and support to ensure efficient and effective case management services.
  • Collaborate with healthcare providers, including doctors, nurses, and therapists, to develop and implement comprehensive care plans for clients.
  • Monitor and evaluate the progress of clients' care plans, making necessary adjustments as needed.
  • Conduct regular meetings with the case management team to review cases, discuss challenges, and provide ongoing training and development opportunities.
  • Ensure compliance with all relevant regulations and standards in the field of case management.
  • Maintain accurate and up-to-date documentation of client records and case management activities.
  • Act as a liaison between clients, their families, and healthcare providers to facilitate communication and address any concerns or issues that may arise.
  • Stay informed about the latest developments and best practices in case management and healthcare to continuously improve the quality of care provided.
  • Analyze the daily activities of the programs to ensure departmental productivity goals are established and attained.
  • Development of productivity reports/dashboards.
  • Evaluate performance of employees, utilized the established productivity criteria.
  • Development and implementation of departmental workflows and templates.
  • Evaluates, evolve and implements improvement plans to follow policies, procedures, and work processes.
  • Contribute to our humble pursuit of excellence by performing various responsibilities that may arise, reflecting our collective goal of enhancing healthcare delivery and being the difference for the customer.
