SUMMARY

In collaboration with patients/families, case managers, physicians and the interdisciplinary team, the Lead Social Worker provides direct psychosocial and practical care including; social work assessments, problem identification, goal setting behavioral health intervention and implementation of care plans. The Lead Social Worker supports continual process improvement in the delivery of clinical social work intervention and care coordination to benefit patient care/outcomes throughout the organization.

  • Serves as a clinical resource expert in a specialty area; provides administrative leadership in the absence of senior leadership and functions as a mentor for others.
  • May engage in academic work including research, grants, publications and education.
  • Assists Director of Social Work in administrative duties.
  • Acts as a resource for more junior Social Work staff and may provide input on performance evaluations.
  • May be required to provide coverage during absences and vacancies.
  • Provides clinical supervision to assigned departmental staff according to COMAR regulations in both group and/or individual settings; may serve as a field instructor for MSW candidates.
  • Provides case consultation to clinical staff for complex psychosocial problems.
  • Collaborates with senior management to assist with policy review, program development, and onboarding of new staff; implements and supports departmental objectives, staffing patterns and performance standards.
  • Completes full psychosocial assessment and plan of care for all patients in assignment area with positive indicators for social work consultation; provides direct psychosocial care and psychosocial transition planning for a full case load of patients in assigned area.
  • Works in a collaborative practice model, facilitating care coordination with the multidisciplinary treatment team for care progression to optimize patient outcomes.
  • Adheres to departmental and regulatory documentation standards and formulates clear descriptions of the problems, goals and interventions. Adheres to timely completion of medical record documentation.
  • Assists with resource referral, capacity determination, legal guardianships, adoptions, psychiatric referral, assessment of abuse/ neglect /IPV and charity care; identifies the need for and facilitates family meetings.
  • Leads or participates in interdisciplinary rounds if applicable for assignment area.



SHIFT: Night Shift

Salary Range: Minimum $38.41 per hour - Maximum $63.42 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.

In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
