JOB DESCRIPTION:

Position Summary: Provides intervention, education, case management services, and linkages to Centerstone programs and services for adults, children, and families. Outreach services are facilitated both in the office and in the field at various community locations.

Essential Functions/Job Duties:

Facilitate the growth and development of continuity of Centerstone Programs. Build relationships with community partners and clients in order to provide linkages to services. Provide knowledge and increase awareness of services available to help clients and families regain health and stability as needed. Educate the community about Centerstone treatment programs and services. Establish working relationships with other community providers to strengthen collaborations with Centerstone. Document and maintain records of outreach services performed. Completes all other duties as assigned.

Minimum Qualifications: High School Diploma or equivalent. Knowledge of local community resources, community partners, and Centerstone programs required. Highly motivated, self-directed, excellent time management, and excellent communication skills are essential. Valid Florida driver's license with a good driving record and current auto insurance.

Preferred Qualifications: Working knowledge of Avatar electronic medical records software, computer skills, and timely data entry skills. Possess awareness of how mental health, addiction, and/or homelessness affect clients, families, and the community. Experience working with individuals with mental health or substance use concerns. Enthusiastic about influencing positive change in our clients' lives.

Special Skills, Knowledge and Abilities: Competency with electronic medical records, computer systems, and documentation. Strong interpersonal communication skills in order to collaborate with team members and maintain professionalism. Accountable and self-directed with daily job duties.

Work Environment/Physical Demands: Outreach Specialists are required to work in the field in different environments and are involved with clients, residents, family members, personnel, visitors, government agencies and staff, etc., in varied settings. Sits, stands, bends, lifts and moves intermittently during work hours. Is subject to frequent interruptions.

Position Type, Expected Hours of Work and Travel: This is a full-time (40hours/week) position with a flexible work schedule to include early morning and evenings. Weekend hours may be required.
