Summary:
A Social Worker - MSW (Inpatient) is a crucial member of the healthcare team, providing comprehensive support and services to patients and their families. They are responsible for conducting assessments, supporting patient-centered care plans, facilitating resources, monitoring progress, advocating for patients, actively listening to their concerns, respecting individuality, building trust, performing risk segmentation, carrying out delegated work, coordinating care, and communicating with payers.

Essential Job Duties: Facilitates Care Progression and Discharge Planning

  • Complete Initial Discharge Planning Assessment in electronic medical record within one business day [goal within 24 hours of admission].
  • Participate in daily Communication and Patient Progression Meetings.
  • Document in electronic medical record if no discharge planning needs have been identified through initial assessment.
  • Conduct face-to-face meeting for all patients to determine the most effective way to coordinate resources and begin discharge planning.
  • Follows Clinical Care Progression to ensure right care, right time, right place, right resources.
  • Communicate medical milestones for transition with the patient/family.
  • Address barriers for scheduled same-day discharges.
  • Identify patient's readiness to discharge based on discussions with the patient/family/care team members.
  • Assess the plan of care and discharge plan to determine needs post-discharge and communicate to patient/family/care team on an ongoing basis.
  • Identify required authorization for post-discharge services and refer to the appropriate post-discharge service provider.
  • Participate in medication resource management for non-resourced patients, as needed.
  • Verify patient's understanding/agreement of plan of care and discharge plan.
  • Refer administrative tasks (e.g., faxing, form processing) to Care Management Assistant.
  • Identify and Present on patients during CCM and collaborate to problem solve issues with complex patients and identify trends.
  • Coordinate with Care Management Assistant by providing updates for discharge planning, payer communication, and transportation.
  • Provide updates to the Care Management Assistant on the discharge plan, discharge delays, and anticipated discharges

    * Master's Degree in Social Work
    Licensure/Certification Requirements:
    * None required
    Professional Experience Requirements:
    * Two (2) years health care experience strongly preferred
    Knowledge/Skills/and Abilities Requirements:
    * * Knowledge of mental health conditions, diagnostic criteria, and evidence-based treatment modalities
    * Strong assessment and diagnostic skills
    * Excellent communication and interpersonal skills
    * Ability to collaborate effectively as part of an interdisciplinary team
    * Empathy, compassion, and cultural sensitivity
    * Strong organizational and time management skills
    * Ability to maintain confidentiality and adhere to ethical guidelines
