Status: Full-time, non-exempt (hourly), non-management

SUMMARY
The People Concern's Westside Outreach Team is seeking a master's level clinical case manager to join our multidisciplinary street outreach team. This team is part of a Westside strategy to combat homelessness. Street outreach aims to identify, engage, and connect, or re-connect homeless individuals to interim and/or permanent housing and supportive services. The team works closely with each client to develop individualized service plans. The team helps enroll clients in public benefits (i.e. General Relief, SSI/SSDI, Medi-Cal), submit housing applications, and access appropriate medical and behavioral health treatment.


ESSENTIAL DUTIES & RESPONSIBILITIES
1. Provide field-based services, meeting currently homeless clients where they are in order to eliminate barriers to services.
2. Assess the mental health service needs of clients.
3. Engage clients in developing mutually agreed-upon goals documented in each client's confidential service plan.
4. Provide risk assessment and clinical evaluations for client's who are potentially gravely disabled and in need of hospitalization and a referral to higher levels of care.
5. Provide crisis assessment and intervention and triage appropriately.
6. Link clients with appropriate medical, mental health, and substance abuse resources.
7. Assist clients in obtaining all benefits to which they are entitled, including applying for and advocating for SSI benefits.
8. Provide opportunities for clients to build a supportive peer community.
9. Build relationships with County Mental Health and other resources to increase collaboration and responsiveness to clients with severe mental illness and addiction.
10. Work collaboratively with public, private, and nonprofit sector groups to provide services, e.g., mobile info stations, public kiosks, hygiene centers, a welcome center, sobering center, etc.
11. Collaborate and communicate effectively with police officers and local hospital staff to reduce police contact and Emergency Department visits by targeted population.
12. Work effectively on an interdisciplinary team and collaborate with Program Manager to ensure high level of clinical oversight.
13. Adhere to an emphasis on customer choice, harm reduction, and psycho-social rehabilitation.
14. Develop and maintain a complete, accurate, and current member file with all funder-required documents
15. Assure that all member paperwork and data collection is complete and up to date in the case management database
16. Other duties as assigned.

QUALIFICATIONS
1. Master's degree in a mental health field (MSW, MFT, PhD, PsyD)
2. Ability to register with California Board of Behavioral Sciences within the first 6 months of employment.
3. Experience working with homeless individuals living with mental illness and/or substance addictions, preferred.
4. Able to work in a high tolerance Harm Reduction model with clients experiencing multiple barriers
5. Able to maintain an effective working relationship with community partners (including police, firefighters, city staff, etc) and maintain and build professional rapport
6. Valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record

WORK ENVIRONMENT
1. Field (may need to travel) and indoor office environment.
2. On occasion walk or drive to different local sites.
3. Regularly required to sit, stand, bend and occasionally lift or carry up to 35 pounds.
4. Will necessitate working in busy and loud environments.
5. Will be exposed to elements like cold, heat, dust, noise and odor.
6. May need to bend, stoop, twist, and sit throughout the day.
