Population Health Specialist

The Population Health Analyst Specialist is responsible for clinical expertise for specific, complex, and/or high-risk member populations designed to meet specific contractual and program-related requirements. This includes establishing, meeting, and continuously monitoring the goals, data, and objectives while maintaining alignment with the strategic goals and objectives for Detroit Wayne Integrated Health Network (DWIHN). The Specialist promotes collaboration between the leaders of each specific member population and other departments by coordinating the activities of Population Health across the DWIHN Network.

Duties include:

Gathering, interpreting, and analyzing data to identify trends, disparities, and areas for improvement

Developing interventions and programs to address the specific health needs of the population

Partnering with community organizations

Measuring and tracking the effectiveness of implemented programs

Working with different computer technologies

Understanding of the healthcare industry within the Prepaid Inpatient Health Plan network

Writing the NCQA Population Health Assessment for DWIHN

Ensures the Expertise in Healthcare programs is aligned with the standards of evidence-based practices for behavioral health and fosters the administration of evidence-based healthcare in a new and innovative way.

Ensure that population health activities comply with relevant regulations and guidelines, such as those related to behavioral health care and Health Effectiveness Information Data Set (HEDIS).

Prepare reports and presentations to communicate the overall population health status and program performance.

Collaborate with all DWIHN departments to ensure NCQA interventions under population health are successfully implemented and rolled out.

Utilize data analytics to stratify patient populations and anticipate healthcare needs.

Demonstrate a dedication to continuous improvement in clinical and cultural settings.

Develop and deliver presentations and training related to population health management.

Skills Include:

Analytical Skills: Strong ability to analyze data, identify trends, and interpret findings.

Communication and Interpersonal Skills: Excellent verbal and written communication skills, with the ability to build relationships and collaborate effectively.

Project Management Skills: Ability to plan, organize, and manage projects effectively, including setting goals, tracking progress, and delivering results.

Technical Proficiency: Using software and tools related to data analysis, reporting, and communication.

Knowledge of Public Health Principles: Understanding public health concepts, theories, and approaches.

Experience in Healthcare: Experience in healthcare settings, such as hospitals, Prepaid Inpatient Health Plan, or managed care organizations.

Experience in writing NCQA performance plans or assessments.

Education:

Master of Public Administration, social work, or related field

5+ years working within the behavioral health system

WORKING CONDITIONS:

Work is usually performed in an office setting but requires the employee to drive to different sites throughout Wayne County and the State of Michigan.
