Overview:

Sentara is hiring a Clinical Pharmacy Coordinator at Obici Hospital

Location: Obici, Suffolk VA

Status: Full Time (40 hrs/wk)

Shift: Day

Weekends: Every 3rd weekend

Holiday Rotation

The Senior Clinical Pharmacy Specialist participates in delivering comprehensive and individualized pharmaceutical care to patients. Assumes leadership responsibility and accountability for daily clinical operations, coordination of clinical workflows, quality and service, strategic planning involving pharmacy, and medication management locally and within the system.

* Manages patient drug therapy in accordance with applicable clinical practice standards and oversees formulary implementation and compliance with P&T-approved criteria at the local facility.

* Serves as a clinical expert, system-level pharmacy representative, and liaison for clinical pharmacy
* Assists with the development of policies and procedures and ensures staff training of clinical policies, procedures, and job aids
* Precepts pharmacy students and/or pharmacy residents as assigned
* Serves as Residency Program Director, Residency Program Coordinator, or Student Coordinator as relevant to the site
* Leads and coordinates site implementation of various clinical system initiatives and local clinical programs
* Develop and implement clinical patient monitoring policies and procedures

* Participates in local and/or system committees related to population health management, quality/safety evaluations, system clinical pharmacy programs, payer contracts, and clinical initiatives as necessary
* Develop drug use evaluation procedures to ensure usage is continuously evaluated to meet patient care requirements
* Develops, implements, and evaluates clinical pharmacy services, protocols, policies & procedures

* In addition to the above job responsibilities, other duties may be assigned

Education

  • Bachelors Level Degree in Pharmacy (Required)
  • Doctorate Degree in Pharmacy (Preferred)

Certification/Licensure

  • Virginia Pharmacist License (Required)
  • Basic Life Support (BLS) (Required)
  • Advanced Cardiac Life Support (ACLS) (Required)
  • Board of Pharmacy Specialties (BPS) Certification (Required)

Experience

  • Minimum 5 years of Clinical Pharmacist experience (Required)
  • Residency Trained (Preferred)
