BASIC PURPOSE: The Project Manager - Implementation is responsible for supporting the planning, coordination, and execution of new PACE (Program of All-Inclusive Care for the Elderly) home health initiatives. This position manages the tactical aspects of implementation efforts across multiple regions, ensuring compliance with regulatory guidelines and alignment with organizational goals. The role involves cross-functional collaboration with operations, clinical, IT, and finance teams, and includes responsibility for tracking performance metrics and optimizing program delivery. ESSENTIAL FUNCTIONS:
- Lead and manage end-to-end implementation of PACE home health programs, ensuring delivery within defined deadlines and schedules.
- Work closely with leadership and internal/external stakeholders to develop detailed project plans, timelines, and milestones; track progress through dashboards and reports.
- Coordinate daily project activities to align with strategic goals and support operational readiness, including opening new PACE centers and launching new services.
- Act as the primary point of contact for project-related communication, ensuring timely, clear updates to internal teams, client stakeholders, and vendors.
- Partner with clinical, IT, finance, and operational teams to ensure cohesive implementation, including effective integration of tools like EVV and telehealth.
- Identify risks, issues, or obstacles early and proactively address them or escalate to leadership to keep projects on track.
- Analyze data trends to identify performance gaps and opportunities for improvement; support data-driven decision-making and recommend enhancements based on lessons learned.
- Ensure all project documentation and processes comply with federal and state PACE guidelines; assist in audit readiness and responses to regulatory inquiries.
- Provide operational guidance and training to onsite coordinators and managers during the implementation period.
- Document and evaluate existing implementation processes; contribute to standardizing procedures, toolkits, and the Implementation Business Review.
- Facilitate regular project meetings and status updates to keep all stakeholders informed and engaged.
- Monitor deliverables to ensure alignment with client needs, quality standards, and contractual commitments.
- Perform other duties as assigned.
Must have the ability to perform the essential functions of the job with or without reasonable accommodation. QUALIFICATIONS:
- 3-5 years of experience in healthcare operations, preferably in home health, PACE, managed care, or long-term care.
- Experience in program or project management, with a focus on cross-functional collaboration.
- Familiarity with PACE regulations and CMS guidelines strongly preferred.
- Strong organizational and project coordination skills with high attention to detail.
- Proficiency in Microsoft Office Suite, project management software, and data visualization/reporting tools (e.g., Excel, Power BI, Tableau, Smartsheets).
- Strong interpersonal and communication skills to work effectively with diverse internal and external stakeholders.
- Ability to manage multiple priorities in a fast-paced environment.
- Analytical mindset with comfort working with complex data and reporting systems.
Education: High school diploma or equivalent required; Bachelor's degree in Healthcare Administration, Business, Project Management, or related field preferred.
