Description The Patient Care Coordinator is a vital member of the Population Health Department, collaborating closely with the Quality Improvement Supervisor and Specialists to support initiatives that enhance patient care and improve quality outcomes. This role involves collecting and reviewing patient health data, patient scheduling, analyzing health risks, coordinating referrals, and managing patient registration (if needed). The coordinator also facilitates patient communications, supports care plans, and helps patients to ensure a positive experience and adherence to treatment plans. Additionally, the Patient Care Coordinator participates in meetings with payors alongside the Quality Improvement Specialist and contributes to programs aimed at improving Pacific Medical Centers' quality measures.
