Under direction of the Registered Nurse, the MHS provides direct patient care and participates in therapeutic milieu.
Does this position require Patient Care? Yes -1Identifies and reports house cleaning problems to Unit Secretary/ND. -2Identifies and reports equipment problems to Unit Secretary/ND. -3Participates in the unit's inventory system, reporting when supplies are low. -4Initiates activities that assist patients in maintaining a clean environment. -5Initiates activities that promote an organized, safe patient and staff environment. -Assists patients in their activities of daily living. -1Identifies patient hygiene needs; initiates and implements interventions to meet these needs. -2Provides for patients' nutritional needs according to unit procedure. -3Assists and reassures patients during tests, exams, and procedures. -4Escorts and supervises patients on and off ground, maintaining patient safety. -Maintains a commitment to professional growth and competency. -1Completes mandatory in-service requirements. -2Identifies areas of strengths and areas in need of professional development with supervisor. -3Maintains competency for treating age-specific patient populations. -4Seeks constructive feedback, regarding work performance, from peers and professional colleagues. -5Participates in critical incident de-briefing. CRC Job Summary Summary Under direction of the Nurse Manager, Clinical Coordinator & Charge Nurse, the Community Residence Counselor (CRC ) provides direct patient care and participates in therapeutic milieu within the scope of defined practice. .Does this position require Patient Care?Ye Essential Functions -Provides direct patient care in accordance with current treatment plan, under the supervisor of the Nurse Manager, Clinical Coordinator and Charge Nurse. -Provides and supervises community activities according to needs. -This may include driving & transporting patients to and from activities in hospital vehicles. -Remains awake and alert, at all times, while on the job. -Assists patients in maintaining and keeping various appointments. -Clear, concise, and timely documentation and shift notes. -Establishes relationships with patients that are helpful and maintains professional boundaries. -Participates in patient rounds, status checks, or teams per program guidelines. -Completes assigned patient care tasks per program guidelines and/or daily assignment sheet. -Works effectively to manage the therapeutic milieu. -Provides patients with direction and supervision as needed. -Follows established hospital & program policies, procedures, and protocols. -Communicates information regarding patient milieu/therapeutic environment to appropriate staff in a timely manner. -Communicates observations related to medication side effects, physical and emotional changes to appropriate medical staff in a timely manner. -Encourage patients in their activities of daily living. -Assesses sleep patterns and assists patients to develop more appropriate sleep cycles notifying clinical team as necessary. -Completes mandatory annual in-service requirements, including but not limited to: CPR and de-escalation. -Attends and participates in rounds and case conferences when appropriate. -Physical ability for participation in patient activities. -Other duties as assigned.
