Job Description:

Nurse Navigator - Oncology Screening (Lung Cancer)
PRN

At Baptist Health, our Oncology team is committed to compassionate, patient-focused care from prevention through survivorship. The Nurse Navigator - Oncology Screening for Lung Cancer plays a vital role in guiding patients through the complex healthcare system with a focus on education, care coordination, and early detection. Working in collaboration with an interdisciplinary team, the Navigator helps patients access timely care and necessary resources throughout their lung cancer journey.

Provides patient navigation, education, and care coordination for lung cancer screenings and related follow-up care. Supports referring providers, manages program workflows, and maintains screening databases to ensure quality and continuity of care. Acts as a liaison between patients, providers, and community partners to improve outcomes and eliminate barriers to care.

Responsibilities:

  • Assesses patient needs including educational, psychosocial, and access-related concerns

  • Coordinates diagnostic studies and referrals for patients throughout the screening process

  • Ensures timely follow-up and continuity of care through database tracking and direct patient contact

  • Maintains and updates lung cancer screening database, ensuring accurate data entry and statistical reporting

  • Collaborates with physicians, radiologists, oncologists, and support staff to guide patients through diagnosis and treatment planning

  • Acts as a liaison between the patient and the healthcare team, offering advocacy and support

  • Facilitates communication with referring physicians regarding patient progress and next steps

  • Participates in multidisciplinary meetings and cancer conferences to support patient care decisions

  • Engages in physician outreach and community education efforts related to lung cancer prevention and screening

  • Promotes access to healthcare and community-based resources

Requirements:

  • Associate Degree in Nursing (ADN) required

  • Must enroll in a BSN program within 1 year of hire and complete the BSN within 5 years

  • Current RN license in the state of Kentucky or a compact state

  • Basic Life Support (BLS) certification required

  • Minimum of two (2) years of nursing experience required

  • OCN certification preferred

  • Strong communication, coordination, and critical thinking skills required

  • Ability to serve a patient population including adults and geriatrics
