POSITION SUMMARY:

Within the Hematology and Oncology Department at Boston Medical Center, the Cellular and Gene Therapy Case Manager works closely with a multi-disciplinary team to provide complex administrative support for the program and provides case management for individuals undergoing treatment with cellular and/or gene therapies.

Position: Patient Case Manager - Cell and Gene Therapies

Department: Stem Cell Transplant

Schedule: Full Time

ESSENTIAL RESPONSIBILITIES / DUTIES:

  • Conducts patient intake and case management for cellular and gene therapy patients alongside treatment-specific Advanced Practice Provider (APP) and/or treating physician:

    • Creates a safe, warm, welcoming environment and encourages engagement in care.
    • Completes a comprehensive status assessment of the patient's insurance, housing, transportation, caregiver/support system, occupation, and applicable social needs.

      • Connects patients to a broad range of services including but not limited to patient navigation, social work, mental health services, legal services, financial assistance, interpreter services, etc.
      • Develops an understanding of insurance prior authorization requirements to submit authorization for the planned treatment.
      • Provides education on housing and transportation assistance offered and completes applicable referrals following initial assessment.
      • Works with the patient to complete FMLA paperwork for the patient and their caregiver, if deemed necessary.


    • Provides patient education resources within the Cellular and Gene Therapy Program, applicable manufacturing companies for offered treatments, and BMC support services. The Case Manager will also provide tools and facilitate connections to these resources.
    • Schedules the patient's program-specific pre-testing and planned treatment, which includes communicating with internal and external parties, and preparing individual treatment calendars.
    • Develops an understanding of the patient's disease to complete pre- and post-treatment reports; including, but not limited to eligibility checklists, admission reports, discharge summaries, etc.
    • Maintains organization of patient-specific shadow charts.
    • Collaborates with program to provide additional patient support, as indicated by the intake assessment.
    • Serves as a liaison between the patient, clinical team, and applicable manufacturing companies.


  • Serves as a key member of the Cellular and Gene Therapy team:

    • Maintains full ownership of the master patient list, master calendar, and internal patient tracker.
    • Facilitates weekly Cellular and Gene Therapy meetings and provides information for clinical review and interdisciplinary patient-focused discussions. Active participation from the Case Manager is required during these weekly meetings.
    • Meets with each patient undergoing cell and/or gene therapy to ensure patient is successfully connected with appropriate resources, services, and providers.
    • Works closely with the Senior Program Manager and Clinical Quality Manager to ensure compliance with the Foundation for the Accreditation of Cellular Therapy (FACT) through standard operating procedure revisions, annual health related outcome reports, and routine auditing.

      • Maintains competency in applicable FACT standards.
      • Takes part in on-site FACT inspection, including inspection preparation and follow up.


    • Works closely with the Data Manager to ensure all patients are approached for the Center for International Blood and Marrow Transplant Research (CIBMTR) data repository and accurate submission of pre- and post-treatment study related data takes place.

      • Case Manager will be enrolled into CIBMTR and BU IRB and trained as a Data Manager.
      • Maintains competency with the CIBMTR manual, including periodic updates.
      • Takes part in routine on-site audits with CIBMTR. Monitors progress on applicable corrective action plans.


    • Performs and maintains all documentation in the electronic chart of patient encounters, and other patient interventions and interactions.
    • Maintains HIPAA standards and confidentiality of protected health information.


  • Program development and administrative responsibilities include:

    • Participates in multidisciplinary/staff/team meetings and activities.
    • Participates in team-based Quality Improvement (QI) initiatives.
    • Participates in program operational meetings; and is actively engaged in developing, implementing, and refining operational protocols and processes as the program expands.
    • Engages in meetings with external CAR T and gene therapy manufacturing companies as needed.
    • Encouraged to engage in learning and growth opportunities including attending weekly Grand Rounds, and pursuing training opportunities with the approval of supervisor.
    • Adheres to departmental/organizational policies and procedures.
    • Adaptable to changing programmatic needs.
    • Performs other administrative functions as assigned by Manager/Supervisor.


*This position has the ability to flex to a hybrid role once employee on-boarding has been completed successfully.

JOB REQUIREMENTS

EDUCATION:

Requires Bachelor's degree (or equivalent combination of formal education and experience).

EXPERIENCE:

Minimum 3-5 years of relevant post-collegiate professional experience required. Previous project coordination also preferred.

KNOWLEDGE AND SKILLS:


  • Excellent verbal/written English communication skills, including excellent grammatical, editing and proofreading skills.
  • Advanced skills with Microsoft applications (Outlook, Word, Excel, PowerPoint, Access) and other web-based applications. Ability to produce complex documents, perform analysis and maintain databases.
  • Ability to work independently and exercise independent judgment.
  • Excellent interpersonal skills necessary to be socially perceptive in accepting and relaying confidential information, communicating policies and procedures, and interacting effectively with a variety of Medical Center personnel and outside personnel.
  • Strong problem solving skills.
  • Proven ability to work with confidential information.
