The Broward County Board of County Commissioners is seeking qualified candidates for a Human Services Coordinator in the HOSS Permanent Support Housing section of Housing Options, Solutions, and Supports division.

General Description
Provides advanced social case management, counseling, and/or coordination of services in an assigned human services program.
Works under general supervision, independently developing work methods and sequences.

Minimum Education and Experience Requirements
Requires a Bachelor's degree from an accredited college or university with major coursework in mental health counseling, marriage and family counseling, counseling education, social work, psychology, gerontology, or other closely related field based upon area of assignment.

Requires one (1) year of direct client social case work/counseling in either an addiction/substance abuse treatment or mental health facility/program, crisis intervention or clinical setting, or in a community resource eligibility center based upon area of assignment.

Special Requirements
Depending upon area of assignment, may be required to work nights/weekends/holidays.

Special Certifications and Licenses
Must possess a valid Driver's License at time of appointment and must be able to obtain and maintain authorization to drive on County business.
Depending upon area of assignment, may be required to have a Certified Addiction Counselor (CAC) Certification.

Preferences
-Masters Degree in a related field.
-Microsoft Excel Certification
-Bilingual in English/Spanish, English/Creole, English/French and/or English/Portuguese.
-Knowledge of homeless systems of care, providers and homeless management and information system.

DUTIES AND RESPONSIBILITIES

The functions listed below are those that represent the majority of the time spent working in this class. Management may assign additional functions related to the type of work of the job as necessary.

This position provides advanced case management, housing navigation, and/or the coordination of intensive intervention service referrals for individuals and families experiencing homelessness.

Interviews customers and families to complete a comprehensive needs assessment to determine customer needs.

Engage customers and families in the development and updating of service plans to address customer needs and to facilitate referral to appropriate treatment or service providers or facilities.

Provide holistic and comprehensive case management services, including intake, assessment, care, and discharge planning.

Screen contacts/customers to determine eligibility for assistance in a fast-paced setting, including obtaining, reviewing, and verifying documents submitted to ensure compliance with Division and funder policies and procedures.

Analyze financial documents, formulate budgets, and calculate monthly income and expenses.

Maintain a case load, including frequent customer contact and field visits.

Connect customers with needed and available community resources.

Create a record and input customer data in the electronic record system in compliance with established time frames.

Process requests for disbursement of funds for eligible customers.

Conduct customer follow-ups.

Provides crisis prevention, intervention, and supportive counseling as needed.

Exhibits knowledge of social work case management principles, policies, and procedures.

Participate in case staffings and consultations.

Exhibits the ability to perform case management activities, maintain related documentation, and participate in team meetings in accordance with regulatory and accreditation bodies.

Conduct outreach and networking services.

Act as team lead as needed.

Data entry into various information management systems.

Participate in professional development trainings/classes on an ongoing basis.

Perform related work as required.

Physical Demands

Physical demands refer to the requirements for physical exertion and coordination of limb and body movement.

Performs sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations.

Unavoidable hazards refer to the job conditions that may lead to injury or health hazards even though precautions have been taken.

Involves routine and frequent exposure to traffic; moving machinery.
