Registered Nurse (RN) Job Objective

The role of the Registered Nurse (RN) is to ensure the health and safety of all residents through ongoing assessments, staff training, and compliance oversight. The RN will provide clinical leadership, ensure staff are trained on current medical procedures and policies, and maintain regulatory compliance at all times. This role requires professionalism in working with residents, families, and staff, a positive work attitude, and the ability to collaborate as part of a team.

Responsibilities and Duties
  • Complete initial assessments of potential residents to confirm needs can be met.

  • Conduct admission assessments within two weeks of move-in and reassess residents regularly.

  • Ensure residents maintain admission criteria; recommend alternative placement when necessary.

  • Collaborate in developing service agreements for new residents; review and update agreements as needed and at least annually.

  • Maintain accurate and up-to-date resident charts, service agreements, and clinical progress notes in accordance with state regulations.

  • Communicate changes in resident condition to physicians, caseworkers, and other relevant personnel.

  • Coordinate resident services with outside providers as needed.

  • Oversee medications, renewals, and physician follow-up regarding medications or medical conditions.

  • Ensure physician orders are signed and implemented within 24 hours.

  • Monitor medication administration practices, addressing corrections and errors with staff.

  • Review resident medication sheets monthly to ensure accuracy and compliance.

  • Document medication disposal per established procedures.

  • Perform supervisory visits within 14 days of admission and at least every 90 days thereafter.

  • Provide training for new staff and ongoing education for all staff, documenting in compliance with state and federal requirements.

  • Supervise medical practices, medication administration, and use of medical equipment.

  • Monitor resident vitals (blood pressure, weights, pulse) and ensure accuracy of documentation.

  • Address and share resident or family concerns with appropriate staff.

  • Assist in the development, implementation, and adherence to policies and procedures.

  • Attend monthly staff meetings and contribute to a positive, team-focused work environment.

  • Ensure emergency procedures are established and communicated.

  • Maintain flexibility to adapt to changes in the assisted living industry.

  • Be available per the on-call schedule to respond to clinical concerns and emergencies outside of regular hours.

  • Ensure full compliance with regulations and participate in state and federal licensure surveys.

Qualifications
  • Current Registered Nurse (RN) license in good standing per state regulations.

  • Minimum of one year of experience in home health, assisted living, or long-term care (preferred).

  • Strong organizational and self-starting skills with the ability to work independently.

  • Demonstrated compassion and commitment to caring for seniors.

  • Strong judgment, problem-solving, and decision-making skills.

  • Exceptional written and verbal communication skills.

  • Experience with computerized resident record systems.

  • Proven ability to work effectively with seniors and their families.
