The clinical pharmacist provides comprehensive medication management services that integrate clinical services, order management, patient/staff education and research. Clinical pharmacists are responsible for improving the patient's experience, safety, and achieving positive clinical and financial outcomes. This position is responsible to an immediate supervisor/manager.I. Major Responsibilities: The following key performance areas (Clinical, Operations/Distribution, Financial, Safety and Regulatory, Research and Education) are performed by the clinical pharmacist: Clinical Performance 2. Maintains and updates a patient profile with demographics (e.g. height and weight), diagnosis, allergies, and current medications. Operations/Distribution Performance Financial Performance Safety and Regulatory Performance Research and Education Performance II. Position Qualifications: License/Certification/Education: Preferred: Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements. ADDENDUM FOR CLINICAL PHARMACIST, TRANSITION OF CARE ROLE Position Summary: The Transition of Care (TOC) clinical pharmacist supports the medication reconciliation process and provides medication management services that integrate clinical services, order management, patient/staff education, and research. The TOC pharmacist is responsible for improving the patient's experience, safety, and achieving positive clinical and financial outcomes upon hospital admission. Participates in and contributes to educational programs, committees, and clinical research. Major Responsibilities: Difference in Qualifications: ADDENDUM FOR CLINICAL PHARMACIST, MEDICATION SAFETY ROLE Position Summary: The Medication Safety Clinical Pharmacist provides comprehensive medication management services that integrate clinical services, order management, patient/staff education and research. Responsible for improving the patient's experience, safety, and achieving positive clinical and financial outcomes. Responsible for all aspects of medication safety at both inpatient and outpatient settings which includes monitoring current practice, identifying opportunities for improvement, facilitation of error prevention strategies, monitoring impact of improvement strategy and developing/revising policies and procedures. Focuses on best practices for the patient and staff, as well as the education/training of all applicable staff. Major Responsibilities: Difference in Qualifications: 2. ASHP Accredited PGY2 Specialty Pharmacy Residency. ADDENDUM FOR CLINICAL PHARMACIST, PALLIATIVE CARE ROLE Position Summary: Responsible for enhancing the quality of patient care within area of Palliative Care. Serves as senior clinical practitioner for a specialty area of pharmacy practice. Provides comprehensive evidence based assessment, planning, implementation, and evaluation of patient care within Palliative Care. Develops system/organizational modifications to support and improve the practice of Palliative Care. Provides consultation regarding pharmacotherapy for symptom management in specialty area of practice.
