Under general supervision of manager, the incumbent assists with the facilitation of daily operations and the creation of process innovations and improvements across the delivery network serving Lawrence and Memorial Hospital, Westerly Hospital and associated off sites.
Responsible for maintaining an interactive departmental program within the Breast Center supporting the department manager, staff, and radiologist(s). Assist in the maintenance of the tracking, monitoring and goals and objectives for the Mammography Quality Control, Quality Assurance Program, Medical tracking, and auditing as set forth per the FDA, MQSA (Mammography Quality Standards Act), ACR (American College of Radiology) and all associated regulatory standards.
Responsible for coordinating patient care activities for select at risk individuals, including but not limited to in-person/virtual/telephonic medical history and pre-procedural screenings, medication education, wound checks, procedure, and recovery education. Assists healthcare consumers (patients, survivors, families, and caregivers) to access appropriate care and navigate through the healthcare system.

Responsibilities

  • 1. Assists in maintaining continuous follow-up and documentation on all abnormal breast imaging exams for 1 year of original exam date i.e., BIRADS 3, 4, 5, 0 as required.
  • 2. Develop and maintain process for management of patient communications and notifications as per the FDA requirements.
  • 3. Provides for appropriate patient and family education and counseling regarding disease and treatment, potential complications, signs, and symptoms requiring immediate attention.
  • 4. Documents patient's progress, compliance, and other essential information in the medical record. Ensures patient's primary care and sub-specialty providers are provided timely information on patient's progress and additional needs.
  • 5. Oversees the correspondence with referring and associated Providers internal and external to YNHHS and associated entities.
  • 6. Maintain interventional breast biopsy database with interpretation of pathology.
  • 7. Participates in ongoing education and networking regarding navigation of patients with cancer.
  • 8. Discuss and oversee need for callback appointments with patients with clinical knowledge and explanation.
  • 9. Complete wound checks as necessary
  • 10. Place intravenous access as directed
  • 11. Assist with contrast reactions
  • 12. Oversees the booking of emergency add-on patients i.e. mammography/breast u/s combo appointments.
  • 13. Research & resolves schedule, patient care and communication needs with associated Providers internal and external to YNHHS
  • 14. Assist in MQSA/FDA inspections at all sites.
  • 15. Work directly with "CBCCEDP" program.
  • 16. Assists manager in being a resource for technical and secretarial staff.
  • 17. Assists in facilitation and coordination of patient flow in the mammography suite, stereotactic room, and breast ultrasound room.
  • 18. Assists in coordination of weekly audit on all un-read mammography & perform or delegate to secretarial staff a physical search.
  • 19. Provide Physician Referral Report every month.
  • 20. Assist in collection of statistical data for statistical analysis, and presentation.
  • 21. Monitors mammographer and radiologist callback rates and informs manager when corrective action is needed.
  • 22. Maintains daily communication with manager and provides outcomes to manager on monthly basis.
  • 23. Assists manager in working closely with PACS administrator for resolution of issues. Work directly with PACS assistants on process creation, training, and implementation.
  • 24. Serves as charge person during manager absences.
  • 25. Assists in the development in goals and objectives in the QC/QA program.
  • 26. Oversee implementation of QA/QC policies and procedures in partnership with the Quality and Safety Coordinator
  • 27. Provide trouble shooting assistance to staff with quality control issues in partnership with the Quality and Safety Coordinator
  • 28. Bring quality concerns to manager and present at staff meeting. Participate in staff meetings.
  • 29. Assist in maintaining ACR re-accreditation all sites.
  • 30. Keep up to date with Medicare Guidelines and coding issues
  • 31. Support monitoring federal and hospital quality standards.
  • 32. Assist in selection and maintenance of mammography reporting system in accordance with MQSA requirements.
  • 33. Participate in outreach programs and breast task force team with manager.
  • 34. Meets performance expectations for Customer Service, Teamwork, Resource Utilization, and Staff and Self Development as outlined in performance review.
  • 35. Assist Radiologist or Clinicians during imaging and interventional procedures when requested
  • 36. Aid in procuring, ordering, storing, and maintaining equipment and supplies when necessary
  • 37. Utilize critical thinking skills in all situations i.e., troubleshooting, equipment, network failure etc.
  • 38. Participates in training breast imaging team/students
  • 39. Prepares and maintains accurate records as directed to ensure correct, appropriate, and timely patient information in accordance with ACR guidelines
  • 40. Participates in developing policies, procedures, and standards of patient care in conjunction with Director, Manager and Medical Director. Participates in developing specific patient care protocols.
  • 41. Participates in ongoing quality assurance activities by collecting and analyzing statistical data and auditing patient records. May assist in preparing grant proposals, writing program reports, and the like.
  • 42. Assists in assuring proper compliance with outside regulatory agencies and assists in preparation and maintaining necessary data collection for regulatory certifications for the center.
  • 43. Collects and reports information to ensure compliance with grant expectations and reporting requirements, if appropriate.
  • 44. Utilize critical thinking skills in all situations i.e., troubleshooting, equipment, network failure etc.
  • 45. Performs other duties as assigned or directed to ensure smooth operation of the department/unit.

    • 46.1 Provides oversight to support staff, ultrasound technologists, mammographers and breast specialists

Qualifications

EDUCATION

Baccalaureate Degree in Nursing required. Master's Degree in healthcare related field preferred. Current CT State RN license and/or eligibility for CT state RN licensure required. Successful completion of National Consortium of Breast Centers Breast Navigation Certification Program within 1 year of hire. BLS certification.

EXPERIENCE

2+ years women's/breast health experience OR three years oncology nursing experience in inpatient, ambulatory, or community nursing required. Case management experience preferred. One to two years of demonstrated leadership/supervisory/management experience required.

LICENSURE

Connecticut Registered Nurse licensure

SPECIAL SKILLS

Strong interpersonal, communication, and organizational skills, both written and oral.
Must be able to multitask efficiently and effectively. Strong facilitation skills.
Knowledge of FDA, ACR, MQSA regulations and accreditation process for Mammography/Stereo/Breast U/S.
Computer knowledge of Word and Excel required Strong analytical, critical thinking and reporting skills.
Ability to work autonomously, efficiently, and effectively under pressure.
