Posting Summary
Rutgers, The State University of New Jersey is seeking a Sr. Care Coordination Specialist within the Epidemiology Cancer Health Disparity Department at the Rutgers Cancer Institute.
The Sr. Care Coordination Specialist supports the improvement of patient outcomes through implementation of evidence-based patient navigation services for cancer screening at ScreenNJ partner sites throughout New Jersey and coordinates daily navigation efforts across the assigned region to support the Patient Navigators and Program Director, Social Services. The focus of the ScreenNJ program is prevention and early detection of lung cancer and colorectal cancer, as well as breast, prostate, cervical, other HPV-related cancers.

The Sr. Care Coordination Specialist provides guidance to some of the most at-risk patients, helping clients to prevent cancer, screen for cancer, and gain access to timely and high-quality follow-up cancer care, as well as ensuring that navigation efforts are coordinated and consistently available to residents of the region. The position provides individualized assistance to patients to facilitate access to cancer screening.

Often the first point of contact with the patient, this position ensures that patients are informed of potential care options and have full access to cancer screening and follow-up options. The position will serve as a key resource for developing and expanding patient access to care resources and overcoming barriers to screening.

The Sr. Care Coordination Specialist also serves as a problem-solving and technical resource for the patient navigators in their assigned region of the state, assisting in resolving complex patient and system issues and coordinating data collection and outcomes tracking to support iterative improvement efforts.

Among the key duties for this position are the following:

  • Provide daily task delegation, work direction, and guidance to the Patient Navigators in the assigned region of the state to assist the Program Director, Social Services in overall coordination of screening navigation implementation, delivery, and iterative improvement statewide.
  • Develop relationships with health care providers within and outside the health center/practice (ScreenNJ partner sites in the assigned region of New Jersey) to facilitate assessment and referral of patients to appropriate cancer screenings and patient access to screening services.
  • Ensure that navigation team members conduct screening navigation activities in line with practice guidelines for screening provided by the ScreenNJ APN and Program Director, Social Services.
  • Facilitate and schedule patient appointments/referrals with appropriate provider(s) for cancer screening and follow-up.
  • Make confirmation calls/emails to patients regarding future appointment dates.
  • Respond to patient inquiries and assist patients with the completion of paperwork.
  • Educate patients on care processes and provide counsel to reduce anxiety and fear.
  • Support internal and community partner quality improvement efforts regarding cancer screening and referrals.
  • Identify and address patient barriers to care such as transportation, insurance, and language barriers, and connect patients as needed to additional resources to support engagement in care. Facilitate patient attendance at scheduled appointments on time and prepared, coordinating transportation when needed.
  • Using Electronic Health Record (EHR) and other standardized systems, track navigation services, record messages from patients, and contribute to clinic tracking workflows.
  • Track appropriate patient-level quality outcomes for program monitoring and evaluation.
  • Adhere to clinic departmental policies and procedures, which include Joint Commission (JC) standards, Patient Safety initiatives, Patient Rights, and Health Insurance Portability and Accountability Act (HIPAA) Privacy standards.
  • Attend and support community-based patient outreach, recruitment, and screening events to assist in connecting eligible patients to risk-appropriate cancer screenings.

Minimum Education and Experience

  • A bachelor's degree in a related field of study (i.e.: psychology, public health, etc.).
  • Equivalent experience, education and/or training may be substituted for the education requirements.
  • 4+ years of experience with public health, social work, health care operations, or similar/relevant work experience.

Preferred Qualifications

  • Professional credential (e.g., LCSW) indicating knowledge and expertise in patient care and complex patient case management preferred.
  • Fluent oral/written Spanish or other non-English language.
  • Previous experience in a problem-solving, patient/client-facing role (preferably in healthcare, as a patient navigator or case manager) preferred.
  • Two or more years of experience in team or project coordination, including coordinating and prioritizing tasks between multiple people and/or multiple projects.
  • Interest in and knowledge of community health care and primary care.
  • Electronic health record (EHR) experience.
  • Experience guiding patients through Medicaid, Charity Care, and other insurance/payment plan enrollment or access.
  • Experience in or with a Federally Qualified Health Center (FQHC) or other clinical setting serving a high proportion of uninsured and Medicaid patients.
