Job Purpose

The overall purpose of the Housing Navigator/Case Manager is to ensure that all housing procured for the Supportive Services for Veteran Families (SSVF) program meets all the standards as provided by the Department of Veteran's Affairs (VA) and the SSVF program. The incumbent will also provide comprehensive case management, financial counseling, and assistance to assist veteran families to become stably housed.

Qualifications


  • Bachelor's degree in social services and or a combination of education and experience of outreach and housing
  • Valid driver's license, current insurance, and reliable transportation is required
  • Minimum two years of experience in case management, life skills as well as conducting assessment and counseling services
  • Experience working with Veterans is preferred
  • Experience delivering services to SUD, COD, or homeless populations is also preferred



Duties and Responsibilities

Case Manager

  1. Delivery of case management and supportive services to homeless or at-risk of becoming homeless veterans;
  2. Maintain case files
  3. Review the Intake Specialist's screen, matrix, categorization, and comprehensive assessment
  4. Determine supportive service needs
  5. Develop a Housing Identification and Stability Plan
  6. Provide housing stability counseling:

    1. Awareness and referral to rental and rent subsidy programs
    2. Awareness and referral to housing assistance programs


  7. Educate participants on supportive service availability and client rights, including:

    1. Fair Housing
    2. Landlord-tenant laws
    3. Lease terms
    4. Rent delinquency
    5. Resolution or intervention of mortgage delinquency
    6. Home maintenance and financial management


  8. Develop and monitor Single Coordinated Care Plan for participants
  9. Request temporary financial assistance after obtaining approval from the supervisor
  10. Make program referrals and establish linkages with appropriate agencies/service providers to assist participants in obtaining needed supportive services, including, but not limited to:

    1. Non-compensation benefits from the VA
    2. Vocational and Rehabilitation Counseling
    3. Employment and Training Services
    4. Educational Assistance
    5. Healthcare Services
    6. Childcare Providers and Assistance
    7. Protective Payee Services
    8. Legal Services
    9. Daily Living Services
    10. Direct Financial Assistance
    11. Emergency Assistance


  11. Execute participation agreements with participants
  12. Maintains accurate and timely records of clients through detailed case noting of assessments, contacts, and progress using required case management forms and maintains required HMIS records
  13. Coordinate with additional staff to provide pick-up/drop-off transportation services and administer bus tickets
  14. Provide financial counseling and assist the participant in developing a personal budget
  15. Create a discharge plan for participants including referral options


Housing Navigator


  1. Develop and implement an individual housing plan for each veteran family
  2. Assist Veterans with completing housing applications and securing housing of their choice
  3. Educate veterans on landlord & tenant rights and responsibilities and how to be a good neighbor
  4. Ensure that Veterans have the necessary items to secure housing i.e. income verification, I.D., etc.
  5. Act as a liaison between SSVF, VA (HUD-VASH), and Veteran families.
  6. Advocate to help address issues and barriers between Veteran and landlord that may prevent the participant to move in.
  7. Assist Veterans with obtaining housing by providing housing leads via; the internet, housing authority, landlord database, etc.
  8. Develop relationships with property management, property owners, and low-income housing programs.
  9. Conduct weekly follow-ups to ensure progress is being made towards the veteran's housing goal.
  10. Provide and support linkages to community organizations to obtain furniture/household items etc.
  11. Maintain communication with the HUD/VASH social worker and veteran.
  12. Maintain up-to-date, accurate, and complete veteran interactions and services records by writing case notes in HMIS.
  13. Request temporary financial assistance after obtaining approval from the Supervisor.
  14. Actively participate in case consultation, staff meetings, training, etc.
  15. Transport veterans/attend to prospective housing unit appointments.
  16. Other duties as assigned.



Other Skills and Abilities


  • Ability to effectively communicate clearly and concisely in both oral and written format. High levels of professionalism with different stakeholders Skill in public relations and partnering practices
  • Intimate knowledge of landlord/tenant rights.
  • Awareness of the "housing first" model
  • Must be flexible, maintain composure in difficult situations and a sense of humor
  • Ability to relate to and interact with individuals from diverse cultural backgrounds
  • Maintain confidentiality of written and verbal information
