Pharmacy Technician

Job Description

Ensure patient safety by accurately documenting and verifying medications during transitions of care.

Job Responsibility

Collecting Medication Histories: Interview patients, caregivers, or use external sources (e.g., pharmacy records, primary care providers) to gather a complete list of current medications.

Verifying Accuracy: Cross-check medications for correct dosages, frequency, and routes of administration.

Identifying Discrepancies: Spot omissions, duplications, or potential drug interactions and flag them for pharmacist or physician review.

Documenting in EMR: Enter verified medication lists into the electronic medical record (EMR) system to support clinical decision-making.

Collaborating with ER Staff: Work closely with nurses, pharmacists, and physicians to ensure timely and accurate medication reconciliation.

Job Qualification

High School Diploma or equivalent, required.
Current Pharmacy Technician Certification required, plus specialized certifications as needed.
Current license as a registered pharmacy technician in NYS, required as of 4/25/21.
3-5 years of relevant experience, required. Hospital and ER experience, highly preferred.
