POSITION SUMMARY The Case Manager work directly with the Home Delivered Meal and elderly service programs as a manager for qualify of services. This role is responsible for ensuring program participants receive the highest quality of care and to assist in navigating the needs of participants. ESSENTIAL RESPONSIBILITIES 
 
     
 Conduct Home Delivered Meal (HDM) assessments and/or Expanded In-home services for the Elderly Program (EISEP) assessments, which include an in-home visit, completion of the NYSOFA Compass, and the completion of the additional EISEP required forms; financial assessment, cost share calculation, care plan, personal emergency response unit request form, and the personal emergency response unit release forms as they pertain to the HDM/EISEP clients.Ensure benefits for which a client is eligible are obtained and maintained in accordance NYSOFA.Meet with clients as directed to complete forms required by New York State Office for the Aging (NYSOFA) and Older Americans Act (OAA) regulations to document visit and determine eligibility and the OAA consumer eligibilityObtain annual physician diet prescription form to be used to verify the appropriateness of the home delivered meal program.Assess nutrition risk and review assessment with registered dietician; referral to registered dietician for consultation if appropriate.Maintain client files, updating the journals per each contact/event, making required day-after calls, 15-day home interviews, 2- month or 6-month calls, respond to and handle-to-completion all client related events during the assessment year (cancellation of service, restarts of service, evaluation for changes in services, termination of services etc).Ensure that all supports and Services being provided are safe and consistent, to the extent reasonable, with the client's needs and program regulations.Manage the process for the coordination of resources and Services.Participate and act as a leader in staff development and trainings as directed.Oversee and Coordinate, as assigned, community linkages for entitlement programs and Services.Lead and Participate in outreach activities as directed.All other tasks/duties as assigned.
 
Qualifications REQUIRED QUALIFICATIONS Education: 
 
     
 High School Diploma or EquivalentBachelor's Degree preferred 
 
 
 Experience: 
 
     
 1 year of working in the Human Services field requiredStrong knowledge of community resources, community services, and the legal/civil rights of seniors
