Social Worker, Pedi-focused Clinician, Spanish-speaking, BWH

The Social Worker is responsible for performing social work assessments and interventions with a predominantly (but not exclusive to) pediatric panel of patients. They are dedicated to patient and family-centered care that values personal self-determination, skilled at managing multiple priorities at once, and engaging in creative, compassionate, and ethical problem-solving. As a member of a multidisciplinary health team, they provide short-term clinical therapy, perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations. Social Workers are responsible for working collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community.

Essential Functions
- Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues.
-Formulates biopsychosocial assessment, disposition, and treatment plans.
-Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
-Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan
Knowledge, Skills and Abilities
- Demonstrates proficient clinical judgment and application of professional values and ethics. Integration of prior clinical experience into current practice.
- Demonstrates proficiency to formulate assessment of risks, crisis intervention, psychosocial assessments, treatment plans, and the documentation of both in a timely manner.
- Demonstrates sound clinical judgment and innovation in advocating for clients while maintaining professional boundaries.
- Demonstrates proficient working knowledge of internal and external resources and refers appropriately.
- Provides a range of evidence-based interventions.
- Ability to advocate, coordinate, and continuously communicate with treatment plans with patients, family and the interdisciplinary team.
- Incorporates social sensitivity.
- If appropriate, utilizes supervision and consultation regularly and appropriately.
- Actively seeks assistance with complex cases and situations in a timely manner.
- Engages in quality improvement projects, uses data to measure progress.
- Facilitate skill-based groups.
- Knowledge, understanding and ability to negotiate and work in a complex organization.
- Demonstrate capacity to effectively communicate findings with a broader audience.
-Adheres to policy and patient safety protocol for session documentation and follow-up
-Demonstrates ability to take initiative, operates calmly when responding to patient crises/dysregulation, and collaborates productively across departments in advocacy of the patient.
-Establishes and maintains appropriate patient-therapist boundaries for safe, effective, and efficient clinical work.
About Brookside Community Health Center:
Brookside is a licensed Community Health Center of Brigham and Women's Hospital and provides multi-disciplinary, comprehensive care with services in a range of medical areas including Adult, Family and Pediatric Primary Care, OB/GYN, Family Planning, Dental/Oral Surgery, Nutrition/WIC, Behavioral Health and Specialty Medicine (Cardiology, Pulmonology and Nephrology).
Brookside also has phlebotomy, financial counseling, and community resource specialists onsite. Each year, more than 12,000 patients from Jamaica Plain, the city of Boston and surrounding areas use these services, producing more than 69,000 visits to Brookside each year.
Most of Brookside's patient population are Spanish-speaking and socioeconomically under-resourced. The health center's services, guided by equity, are provided by a bilingual English/Spanish medical and professional staff.
Brookside is a teaching site for Harvard Medical School, Mass General Institute of Health Professionals School of Nursing, Harvard Dental School, Boston University Dental School, as well as other Boston area health care professional programs.

QUALIFICATIONS:

Education: Master of Social Work Degree (MSW) or Master of Counseling/Psychology/Related field from an accredited program from an accredited program required.

Licensure: Requires current Massachusetts Licensed Independent Clinical Social Worker (LICSW) or current Massachusetts Licensed Mental Health Counselor (LMHC)

Language: Native or near-native fluency in Spanish preferred

SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:

  • Demonstrated flexibility, autonomy, and capacity for necessary decision-making.

  • Extensive knowledge of current social work theory and practice.Ability to conceptualize, interpret and discriminately apply theory to practice.

  • Good clinical skills in assessment, crisis intervention, and evidence-based behavioral health treatment.

  • Ability to make appropriate decisions under pressure, to exercise good judgment, and to take initiative and to meet established productivity standards.

  • Sensitivity to a variety of cultures, beliefs, sexual orientation, and lifestyle.

  • Evidence of excellent patient interaction and staff collaboration skills.

  • Ability to communicate well, both orally and in writing. Good planning and organizational skills.

  • Highly developed interpersonal skills to interact effectively with multidisciplinary team members, both at the health center and with the larger community resources, and to interact with patients and their families.
