Position Summary: Success in this role will perform general Pharmacist duties, including but not limited to: preparing medications, completing final check on filled medications, review and approval on data entry of prescriptions, monitoring patient drug therapies and drug utilization reviews.

The successful candidate will report directly to the Pharmacist in Charge. The position is in Roanoke, VA.

Other Key Relationships: Pharmacy Customer Service, New Customer Enrollment, Documentation, Patient Support, and Living Connected/Clinical Services.

Responsibilities

    Prepares and/or oversees the preparation and dispensing of prescription medications to patients
  • Makes decisions regarding generic substitution of prescribed pharmaceuticals
  • Makes outbound or takes inbound telephone calls to assist in various aspects of patient care and/or product distribution to/from practitioners, members, and pharmacies. These calls include, but are not limited to, comprehensive medication reviews, targeted medication reviews, call to practitioners about product awareness, and prior authorizations.
  • Receive physician prescription orders and evaluate the appropriateness of these orders for dosage, drug choice, potential drug interactions, route of administration and stability
  • Supervise the filling, receiving, and dispensing activities involved in providing finished product to the patient
  • Ensure that the patient chart is complete for allergies, current and past medical history, and concurrent medications (including over-the-counter or home remedy medications)
  • Provide patient counseling, education, and monitoring
  • Ensure safety of patients and personnel by exercising good pharmacy practice in the process of drug therapy instructions to the patient
  • Determine remaining doses on hand to manage inventory and avoid waste
  • Provide clinical interventions
  • Assess responses to therapy, patient compliance, and ongoing review of drug regimens; report and manage adverse events
  • Assist the physician, nurse and patient with clinical and technical support
  • Document all communication with physicians, nurses and patients in the patient chart
  • Assist in the maintenance and upkeep of all necessary paperwork required by state and federal agencies as well as the Joint Commission and other accreditation entities.
  • Assist the Pharmacist in Charge in implementing new procedures
  • Assist in the training and supervision of Pharmacy Technicians, and other pharmacy personnel
  • Participate in Performance Improvement Projects
  • Assist in providing in-service programs to clinicians and pharmacy personnel on any aspect of drug therapy
  • Attend and/or complete education and training programs
  • Review literature and other materials pertinent to the practice of pharmacy
  • Complete all competency/skills assessment requirements
  • Participate in on-call schedule if business needs warrant
  • Perform other tasks as determined by the Pharmacist-In-Charge and pharmacy leadership
  • Provides requested information, answers questions and can respond to requests from patients and/or medical practitioners in person or by telephone related to drug indications/contraindications, dosage, drug interactions, and side effects; provides other drug and pharmaceutical information as appropriate
  • Establishes, maintains methods and manner of storage and recordkeeping systems to provide for safekeeping of pharmaceuticals
  • Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards
  • Maintains a high degree of confidentiality at all times due to access to sensitive information
  • Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department
  • Follows all Medicare, Medicaid, HIPAA, and Private Insurance regulations and requirements
  • Complies with state and federal drug laws as regulated by the state board of pharmacy, the drug enforcement administration, and the food and drug administration
  • Maintains all required pharmacist state licensure resident and non-resident

Qualifications

  • Must be a graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
  • Current Pharmacy license in applicable state. License must be maintained as current and in good standing.
  • Ability to continue to obtain licensure in additional states as required
  • Knowledge of drugs and their indications, contraindications, dosing, side effects, and proper administration
  • Knowledge of patient care charts and patient histories
  • Ability to maintain quality, safety, and/or infection control standards
  • Knowledge of related accreditation and certification requirements
  • Ability to communicate technical information to non-technical personnel
  • Knowledge of policies and regulations pertaining to the disbursement of pharmaceutical drugs
  • Knowledge of clinical operations and procedures
  • Ability to perform basic patient assessments and referrals
  • Strong interpersonal relation skills
  • Demonstrated skills using Microsoft Office Suite (Word, Excel, PowerPoint)
