JOB SUMMARY

The Housing Coordinator Mental Health Specialist reports to the Supervising Personal Service Coordinator and is responsible for the leadership, coordination, development, and management of all housing activities.

ESSENTIAL JOB RESPONSIBILITIES



  1. Work collaboratively with Turning Point, the Housing Authority, Independent Living Association, Supportive Housing, and other related agencies to further develop housing opportunities for clients.
  2. Implement procedures for sober living and residential treatment services requirements and link clients to these placements as appropriate.
  3. Develop and maintain community relationships with local land lords, housing purveyors, Independent Living Association and agencies serving residents of the program.
  4. Maintain housing files, complete and submit all documentation and billing as it relates to client housing, and submit applicable reports to corporate office in a timely manner,
  5. Assist all clients in referral and linkage to the appropriate level of housing.
  6. Monitor housing placements to ensure appropriateness.
  7. Train staff the importance of monitoring clients to ensure that the housing placement is at the appropriate level.
  8. Maintain appropriate case recording documentation as per the Rehab option Medi-Cal guidelines, case recording and other contract guidelines.
  9. Plan, coordinate, and provide direct services for the Severely Mental III (SMI) clients. Provide direct services, including case management, to clients. Such as services may include group counseling, individual, and other forms of direct service as outlined in the Individual Service and Support Plan (ISSP) and Plan of Care.
  10. Ensure that the housing activities are operated in accordance with Turning Point program policies and procedures.
  11. Maintain cooperative working relationship with funding agencies.
  12. Collaborate with the Assistant Program Director / PSC Supervisor and case management staff to develop policies and practices that allows residential and case management components to function in a unified and cohesive manner.
  13. Provide the Assistant Program Director/PSC Supervisor with timely reports on any unusual occurrences that might impact the overall program operation or the health and safety of clients or employees.
  14. Complete the minimum training hours annually per Turning Point training requirements.
  15. Think and act quickly and efficiently in emergencies.
  16. Will drive on company business and transport consumers as needed.
  17. Physical presence in the office is required.


ESSENTIAL JOB REQUIREMENTS



  1. A Bachelor's degree in Social Work, Counseling. Psychology or related field.
  2. A minimum of one (1) year of experience in management of supported housing or rehabilitation for difficult to serve families with barriers to successful independent community life is preferred.
  3. Knowledge of best practices regarding the treatment of clients with Severe Mental Illness (SMI) and Dual-Diagnosis individuals.
  4. Knowledge of laws and regulations pertaining to levels of client housing.
  5. The ability to be certified in emergency first aid procedures and CPR is preferred.
  6. Communicate effectively in written and spoken English.
  7. Ability to operate within Electronic Medical Records (EMR) system.
  8. Intermediate computer skills in Microsoft Word, Excel, Outlook and PowerPoint, Office 365, Teams, Zoom and other communication/meeting platforms.
  9. Possess a valid driver's license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.
  10. The ability to pass a pre-employment criminal background check.
  11. The ability to pass a pre-employment drug screen and TB test.

      • Monolingual: Class 135 ($22.21 - $29.23/hour)
